Background- The Quality and safety education in nursing project has three phases to develop the nursing core competencies we will learn about today. In an effort to ensure the nursing profession was keeping pace with the national expectation to deliver high quality, safe, effective, and patient- centered care The Robert Wood Johnson Foundation (RWJF) worked with stakeholders to jump start the QSEN project. Dr. Linda Cronenwett began Phase 1 by determining that all nurses need specific knowledge, skills, and attitudes (KSAs) to deliver high quality, safe and effective patient centered care. This model provides nurses the ability to continuously improve healthcare within the system they work.
Phase II the QSEN faculty, a National Advisory Board, and 17 leaders from 11 professional organizations representing advanced nursing practice defined graduate-level quality and safety competencies for nursing education and proposed targets for the KSAs for each competency (Cronenwett et al., 2009).
Finally in Phase III, RWJF funded the American Association of Colleges of Nursing (AACN) to promote educators to instruct entry level nurses about the six QSEN competencies. However, it didn’t take long to expand these core competencies to all levels of nursing education, especially with the Institute of Medicine’s report in 2011, about the future of nursing. Now, the healthcare reform has and will continue to cause multiple changes in the delivery of care, and the number of Americans with access to this care; therefore, it is important all nurses understand these core competencies and can apply them to their everyday practice. This is why you are here today, to advance nursing practice and be the future leaders of healthcare.

Interestingly, Massachusetts has been reforming healthcare since 2006 and has created their own nursing core competencies. (CLICK) There a 10 MA nursing core competencies that include all six of QSEN core competencies. The goal for these competencies was to establish standardized outcomes of education using these ten core competencies to improve nursing care. Today, I’m going to focus on two of these competencies: (CLICK) Evidence based practice and (CLICK) Informatics.

Evidence-Based Practice (EBP): As defined by QSEN: Integrate best current evidence with clinical expertise and
patient/family preferences and values for delivery of optimal health care. NOF and MA adapted a very similar definition and approach for EBP competencies from QSEN.
As outlined in QSEN these are the core concepts needed for establishing evidence based practice as a core competency in nursing. There is knowledge, skills and attitudes described to apply this core competency to practice. Let’s quickly go through each one.

EBP application. We practice in community care with all ages. How can we apply EBP to our nursing practice? Let’s start by thinking of a clinical problem. Often we encounter issues with medication adherence. Using the knowledge, skills and attitudes about EBP walk through the steps as a group. Develop a question, Search, Critical Appraisal, Summarize, Note Gaps,
Address Gaps, Translational to practice, Analyze affect, Evaluate, and Promote EBP.

Informatics
Informatics application. In our setting, how can we apply the knowledge, skills and attitudes we have about informatics to our nursing practice? Continuing to focus on medication adherence, what about the use of medication dispensing electronic devices and if medication not dispensed by a certain time an alert goes off or a phone call is made to check on patient. We could use our EBP knowledge and develop PDSA to determine the benefits and limits of this information system. Apply current regulatory requirements and determine the data needed for this to be effective. Make sure patient information is safe guarded. Explain how this would empower the patient. Could this have interoperability?

9. Nursing Core Competencies
Following the Institute of Medicine reports dating back to 2001, to Err is Human to 2011 Future of Nursing many stakeholders, professional organizations, and nursing leaders have stepped up to develop nursing core competencies to ensure nursing practice is using the highest quality, safe, effective patient centered care to be offered. Along with the other competencies Evidence Based Practice and Informatics have key concepts associated with nursing knowledge, skills and attitudes to continuously improve the care we provide within our community healthcare setting. Having an understanding of the concepts and how to apply them to practice will improve your nursing care with an overall goal of having a standardized outcome of practice for all nurses nationwide.
