Meet Annie Annie, mid-70’s, Alzheimer’s disease, chronic kidney disease, CHF and diabetes Hospitalized 5 times in 2 years Lives alone PCP referred her to MTM Pharmacist helped with: Changing injections to safer oral diabetes medications Creating a plan for Annie to weigh herself daily and take extra diuretic medication if she notices weight gain to reduce swelling and make breathing easier Helping Annie enroll in mail order program which sends refills at the same time each month Teaching Annie’s brother how to help set up her medication box Helping Annie understand why she needed an antibiotic that she had stopped taking for a chronic hip infection Result: no ED or hospitalizations four months after meeting with MTM pharmacist

When consumers don’t take medicine correctly, preventable health issues turn into chronic diseases, resulting in poor health outcomes and unnecessary health care costs. Non-adherence leads to, $290 billion wasted health care dollars annually 125,000 deaths annually 10 percent hospital admissions 23 percent nursing home admissions As an integrated system of care and coverage, HealthPartners has the unique ability to view care, from perspectives of both the payer and provider. By focusing on optimizing medicine use, we’re also able to look at lowering total cost of care. Our approach, Medication Therapy Management (MTM) helps connects patients with specially-trained clinical pharmacists to help them get the most from their medicine. MTM identifies gaps in care to achieve optimal health outcomes and reduce costs by making sure that patients: are prescribed the correct medicine can afford the medicine they’re taking take their medicine correctly are getting the right results and that the medicine is working

Our comprehensive medication management approach is to provide access to the support our members need — when they need it and for as long as they need it. This team based approach The goal is to understand the patient’s needs and their health goals, so we can support them in achieving their best health. In every visit we want to evaluate if the medicine is The right medicine/there’s a reason to take it Effective Safe Convenient (cost, adherence) Doctors and pharmacists work alongside one another to focus on care needs. While the doctors diagnose a condition, the MTM pharmacist is the expert when it comes to medicine. Often consumers are confused by their medicine and need a partner to walk through the complexities of side effects, interactions, specific directions and costs. While there might not be time to understand these details during an office visit, a pharmacist can work with their patients to get a deep understanding of their medicine. Pharmacists are viewed to be one of the most trusted care professionals and combined with their expertise in medicine, they can support patients in maximizing the value of their medicine. Convenient

A study was done at a large, government employer. HealthPartners studied the effects of MTM with a group of about 600 people. The program had incredible results. Optimal Diabetes Care achieved in 67.1% of MTM participants vs 37.5% in control (8 years). This has since been expanded across their entire population. Now participants are aging into medicate healthier than they would have. 37% control

Using the Medication Therapy Management program has produced overall savings among eligible members in the elevated-risk and high-risk groups. For members having medicine-related problems we have seen: $1,400 savings per year with elevated members at an elevated risk $4,500 savings per year with members at a high-risk These results are based on a moderate/high-risk commercial population. While we can’t track Medicare results, we have every reason to believe we would find the same numbers.
