Poor CRF refers to functional aerobic capacity (FAC) <80%, reduced CRF to FAC 80% to 99%, and normal CRF to FAC ≥100% predicted. All models are fully adjusted for age, sex, diabetes mellitus, hypertension, obesity, current smoking, and poor CRF. Hazard ratios for cardiovascular death are compared by the Z‐score method.
