Meeting Objectives Understand how Care Navigators can signpost patients/carers in early signs of dementia Appreciate the care pathway for older adult mental health services to support patients/carers throughout their journey Apply new knowledge to scenarios seen in practice - highlight the care navigator’s role

Older Adult Mental Health
Includes the following conditions: Dementia Post Stroke Alzheimer’s Disease etc Parkinson’s Disease and its complications Depression

Dementia Advisor Supports patients/families/carers at pre-diagnosis also Arrange visit to meet pt & annual follow up Continual contact for approx 6 mths Current 1 year pilot in EBBI locality – advise on memory loss Tel: /

Admiral Nurses Care Navigators can refer relatives/carers into this service for support & guidance with established/probable patient diagnosis Local to Worcestershire Ageless service Available to any patient registered to Worcestershire GP, regardless of relative’s location. Tel: (triage) Mon to Fri 10:00 to 14:00

Early Intervention Dementia Service
GP Referral: 8 weeks until assessment Offers pre-assessment counseling Available to all non-urgent (low risk) patients Service offered to patients of any age 1st appointment with a nurse; comprehensive history take; doctor to complete cognitive assessment

DRIVING RISK Should there be a concern regarding an older adult who drives – professional obligation to report to DVLA This can be reported anonymously

GP Referral Worcester: Wychavon: Malvern: Pts over 65yrs Support care for urgent (high risk) patients Addresses capacity assessment Includes – depression, anxiety, psychoses

Onside Advocacy Charity organisation
Provide IMCA: independent mental capacity advocate (Mental Capacity Act) to safeguard vulnerable patients without capacity when ‘unfriended’ and serious decisions need to be made about medical or housing issues May visit practice to access patient notes Tel:
