27.7% or 6,071 above the age of 65 7.3% or 1,600 below the age of 65 are disabled (United States Census Bureau , 2015) Increase need for specialized services close to home 85% of US adults >65 have chronic conditions Increased possibilities of complications and ability to care for themselves (Michigan Department of Health and Human Services, 2016) Most home care agencies are based in Traverse City Based nearby represents commitment to the community

Holds a current RN license in the State of Michigan Professional and comprehensive nursing care within their scope of practice Provide patient focused skilled nursing care by: utilizing the nursing process of assessment, planning, interventions, and evaluation Following physician and advance practice professional orders, nursing interventions, and TenderHeart policy and procedure.

Holds a current RN license in the State of Michigan Responsible for coordinating the interdisciplinary plan of care for the patient in a home care environment Coordinates services and resources for the patient within the community Functions as a patient advocate Ensures home care personnel are providing the necessary care, while involving the patient’s family.

9 Job Descriptions Physical Therapist Occupational Therapist
Graduation from an accredited school with a Master’s Degree in Occupational Therapy. Hold a current Occupational Therapist License in the State of Michigan Development and rehabilitation of home health care patients that require therapy for: Mental, emotional, and physical disabilities. Responsible for planning and directing medically prescribed occupational therapy. Physical Therapist Graduation from an accredited school with a Master’s or Doctor’s Degree in Physical Therapy. Improve and manage movement/mobility and pain Improve patient function and independence Planning and administering medically prescribed therapy.

Works under the direct supervision of the manager of the home care office.
