Physiotherapy (Week One- Three)
One to One Appointment What matters to the patient? Building on Personal Goals Chronic Pain issues.

Nurse/Dr (Week One to three)
One to One Appointment Lay Summary- Optional How are you getting on? What different? Where do you want to be? Personal goals are set which we work towards

Pharmacy (Week One to Three)
One to one Appointment Education Empowerment Medicine Reconciliation Identifies any ongoing gaps.
