Code Sheet Important codes for copier, computer, mail/copy room access, etc. Do not give out information to students Parking pass (free parking) Complete form today and return to me No designated parking for faculty/staff, can park in any non-designated zone Safety Complete online readings, review with supervisor, return form to Faculty Support FERPA Law and regulations on maintaining and using student records, which are the Family Educational Rights and Privacy Act (FERPA) of 1974 (Public Law ), as amended; 34 Code of Federal Regulations, Part 99; Oregon Revised Statutes , and Oregon Administrative Rules 580- Division 13 and 576-Division 20. Records Retention Requirements (Faculty Only) Part-time faculty: A copy of final grades must be turned in to Faculty Support at the end of the term Instructors duty to retain any exams or papers not returned to students for one term after completion, two terms for a Summer course Please refer to Record Retention sheet on our website under the Faculty/Staff Orientation section ONID sign-up is used to communicate with students and for Faculty Support to communicate with you You must sign-up in order to login to computers and use Canvas and Online Services September 18, 2018

4 Classroom Items: Term Classroom Schedule OSU Syllabus requirements
The classroom schedule will be ed to you a week before classes and also hung in the main bulletin boards Classroom keys will be placed in your mailbox two days before the first day of class Please make sure to lock the door at the end of each class OSU Syllabus requirements Review with program lead Class Lists Login to MyOSU, select the Faculty & Staff Tab, then select Advisors & Instructors, then View Class Lists Final exam schedule Current schedule here: Follow as closely as possible, please ask if questions See handbook for Finals Week Policies Entering grades and grade policy Grades must be entered by the instructor by 5pm the Monday following finals week in Online Services; watch your ONID for reminders Please refer to our Faculty/Staff Orientation webpage for grade entry details You will need your ONID username and password September 18, 2018

6 Campus Items: Campus closures In case of an emergency Weapons policy
Will be determined by 6am the day of and will be posted on our website For more information refer to In case of an emergency Evacuation meeting site for CH is in the Barber Library Parking Lot and for GRC is the upper level parking lot towards the South building During an emergency situation, faculty are not automatically “in charge” and do not determine whether students stay or leave the classroom or building Weapons policy No weapons are allowed on any campus. A concealed weapons permit does not allow an individual to possess any kind of weapon. Smoking policy CH – no smoking within 25 feet of building GRC and all OSU Property – Smoke free Mandatory Reporting Effective January 1, 2013, employees of Oregon higher education institutions are considered by law to be subject mandatory reporters of child abuse. Drop in Computer Workspace (part-time faculty) First come first serve GRC 244 CH 226 Mailboxes Your mailbox will be either in CH 225 or GRC 121, depending on where your office is located, or where you teach (part-time faculty) The mailbox rooms are locked, but you will be provided a code at orientation, or you can ask the Enrollment Services desk in CH 223, or the front desk in GRC 225. Non-employees and students are not allowed in the mail/copier rooms. Copiers Copiers are in each mailroom, you will be given a copy code at orientation or you can obtain the information from Enrollment Services in CH or the GRC front desk Room reservations For room reservations please refer to our webviewer online. 8 Payday Information: A monthly online timesheet will need to be submitted each month. Please refer to the timesheet instructions in our handbook Payday is once per month, on the last working day of the month. The first check will be a manual check…..and it may be late. Direct deposit will not be active for the first check, and perhaps not for the second check either. Sign up for direct deposit in Online Services. Instructor salary is based on the working days in a month. Therefore, if the Fall term contract starts on 9/16, the September paycheck is for roughly ½ month. If you work 9 days in the Summer term, partially in June and partially in July, you will get paid on the respective payday for the working days in each month.
