1 BAT Training Guide Desktop Version

2 As part of the First Advantage FormFox join initiative, donors will be coming into your location with an authorization form for a BAT and a drug screen, click on the authorization button.

3 Once you enter the authorization number, you will see the patient’s record. Clicking on the patient’s record will produce this pop up below which informs you that these patients will have both a BAT and a UDS, so be sure to complete both services in FormFox.

4 Once you click okay on the pop-up window you will notice two records
Once you click okay on the pop-up window you will notice two records. Choose the ALC record to do the BAT.

5 First you will verify the donor information then click capture Donor Signature.

6 Have the donor sign on the signature pad

7 Select the test device from the drop down menu
Select the test device from the drop down menu. Record the reading date/time, test #, and the screening level. Then capture the technician signature.

8 Have the collector sign

9 Click the Print ATF button

10 Print

11 To end the test, click finish

12 If a confirmation test is required, have the donor wait 15 minutes
If a confirmation test is required, have the donor wait 15 minutes. Then choose the test device, provide the reading date/time, the test #, and the confirmation level. Then click on submit confirmation.

13 Have the collector sign

14 Once the technician has signed, click on the print ATF button.

15 Print

16 To end the test, click finish
