Template = ClassA_C Declaration Template.xlsm The is equivalent to SDoC. Refer to IPC Guidance DOC

2 Locate the Template The revision controlled Master is available in MyWorkShop, IPC1752A Templates and Instructions, DOC Download the ClassA_C Declaration Template.xlsm. Alternately, you may find it also at; Supplier Support Folder; ESMART Support Central; Don’t use the IPC1752 v1.1.PDF. This is old and should be used only for very limited declarations. Use of this Template is preferred to a Supplier Letter or PDF image, which requires our team to transcribe your letter into the XML.

3 Logistics of completing the Template
The file name must remain ClassA_C Declaration Template.xlsm. If the name is changed then the button will not function as desired and there will be error messages. It is suggested that you make a backup copy of the file immediately after receiving it: ClassA_C Declaration Template Backup.xlsm. Then if you potentially damage the working copy you have a good replacement just by changing the file name. Then as you work on a declaration, after completion of the XML generation, rename the file the name with a unique name to save the results for your records. Some people keep a copy to the template with the Header completed and use it as the starting point for the next declaration. This saves reentering the Header information again. The Class A declaration completes the Declaration Portion Columns A-K. While the Class C declaration continues with data as appropriate out to Column W.

Disclosure Date: (MMM-DD-YYYY format, e.g., ‘Jun ’) GE Part #: copy our number as it is given (e.g., ‘ ’) Manufacturer: Your Company Name (free text, e.g., ‘Acme’) Mfr ID: (Manufacturer ID in ESMART tool, e.g., Dunn’s Number OR Mfr name) Part Description: (free text with maximum of 250 characters) Manufacturer Part #: (e.g., ‘0603-5A-471-JAT-XA’) Item Weight: (numeric value only without units, e.g., ’10.00’) UOM: pull down with mg, g, kg (metric units only) Unit Type: pull down with Each, Meter, m^2, m^3, cm, cm^2, cm^3, Liter(part unit, e.g. ‘Each’) RoHS Declaration: pull down with 6 different declarations (1=RoHS, 2= NOT RoHS, 3=RoHS needs exempt Lead in Solder, 4=RoHS with Exemptions, 5=Obsolete, no information, 6=Unknown, no information available.) RoHS Exemption(s): Many in Pull down including medical (See “Exemption Rules” Tab for details) Note: Required fields highlighted in green Declarations One part per Row, up to 487 parts. Stop at Row 500, where you see the Red field.

7 Additional Supplier Input.
There are addition fields that may be appropriate to complete; If the part that data is requested for is NOT RoHS please indicate this in Column J and if there is a replacement, please indicate this in Column X. This will reduce the chances of the replacement moving to a different Supplier. Please indicate availability of the replacement in Column Y. If there are other comments or remarks to be transmitted for the part, please indicate these in Column Z.

8 Generate XML File What happens when you Click on the “Generate XML File” Button A macro checks all of the Required Fields and reports found errors. ALL ERRORS must be cleaned before the output file is generated. This will reduce the exchanges asking for data cleanup after sending to GEHC. See the example below. When all of the content passes the error checks the XML File is generated. This is computer readable output (an image) of what you entered into the Excel sheet. The XML file will have the name of Supplier and the part number. One file for each row or part number (e.g., Acme_sadfasdfasdf.xml). Depending on your computer setup, the generated file will be in your ‘My Documents’ or the folder that contains the Template file. Send the XML file to GEHC, they will add the declarations into their Tool.

9 Supplier Declaration Statement for Class A_C
Supplier certifies that it has met the GEHC Environmental Requirements defined in GEHC document GSP. Supplier certifies that it gathered the information it provides in this form concerning RoHS restrictive substances using appropriate methods to ensure its accuracy and that such information is true and correct to the best of its knowledge and belief, as of the date that Supplier completes this form. Supplier acknowledges that Company will rely on this certification in determining the compliance of its products with European Union member state laws that implement the RoHS Directive. Company acknowledges that Supplier may have relied on information provided by others in completing this form, and that Supplier may not have independently verified such information. However, in situations where Supplier has not independently verified information provided by others, Supplier agrees that, at a minimum, its suppliers have provided certifications regarding their contributions to the part, and those certifications are at least as comprehensive as the certification in this paragraph. If the Company and the Supplier enter into a written agreement with respect to the identified part, the terms and conditions of that agreement, including any warranty rights and/or remedies provided as a part of the agreement, will be the sole and exclusive source of the Supplier’s liability and the Company’s remedies for issues that arise regarding information the Supplier provides in this form.
