FAA INSPECTOR’S ROLE

2. GOALS / OBJECTIVES
PREVENTION: To prevent an incident from happening in the first place that may result in the loss of life; damage to property; or damage to equipment.
AWARENESS: By ensuring that the airport operator /user is aware of his/her responsibilities and location at all times.
TRAINING: Ensuring that the Airport Operator provides the proper training to all employees and complies with 14 CFR PART 139.

6. VEHICLE / PEDESTRIAN INCIDENTS
(PAST INCIDENTS)
RUNWAY INCURSION, XYZ AIRPORT , NY VEHICLE
At 0946E, two vehicles crossed runway 31 without ATC authorization and as a result, XXX Air flight 4831, a B190, was instructed to cancel the take-off clearance. The closest proximity was estimated at 6,500 feet lateral.
RUNWAY INCURSION, XYZ AIRPORT, PA VEHICLE
At 0901E, a XYZ Airlines maintenance tug vehicle was observed crossing the end of runway 17 behind landing traffic without ATC authorization. There were no conflicts reported.
SURFACE INCIDENT, XYZ AIRPORT, NY VEHICLE
At 1215E, a gray pick-up truck was observed crossing taxiway Foxtrot onto runway 10R/28L and exited runway at taxiway Delta without ATC prior coordination or approval. The airport ramp manager was notified of this incident. There were no other conflicts reported.

7. VEHICLE / PEDESTRIAN INCIDENTS
(PAST INCIDENTS)
SURFACE INCIDENT, XYZ AIRPORT, PA VEHICLE
At 1537E, an Airport Operations vehicle was instructed by the ATC to hold short of runway 28R to await the arrival of XYZ Int’l Airlines flight 2551 The vehicle was observed proceeding onto runway 28R without authorization. The ATC instructed the vehicle to return to the approach end of runway 28R. No other conflicts were reported.
SURFACE INCIDENT, XYZ AIRPORT, NY EQUIPMENT
At 1955E, men and equipment were left on taxiway “Y” without the use of a two-way radio. The men and equipment failed to yield to N----- an FA05. As a result, as per the General Aviation Incident above, the aircraft was damaged when it struck some equipment on the taxiway.
SURFACE INCIDENT, XYZ AIRPORT, WV VEHICLE
At 1310E, Mobile 10 a fuel truck crossed active runway 3 without ATC authorization. There were no other conflicts reported.

8. VPD NOTIFICATION PROCEDURES
Air Traffic Control Tower (ATCT): notifies the FAA Regional Operations Center and other FAA agencies of all incidents by filing a Preliminary Report (FAA Form )
FAA Regional Operations Center (ROC): will review and post this information on the FAA Administrator's Regional Daily Operation’s Report.
FAA Airports Division (ACSI): once the report is received, the FAA Airport ACSI is responsible for the Investigation of the VPD and will notify the Airport Owner of the incident through a Letter of Investigation (LOI).

9. NOTIFICATION PROCEDURES - cont
Airport Operator : Responds to the Letter of Investigation to explain the circumstances that led up to the incident and what actions the airport is taking to prevent any further incidents.
FAA Airports Division: Reviews the airport’s response and makes the final determination as to whether the airport has taken the appropriate action. If the airport’s response is satisfactory, the LOI is closed out with the airport. FAA From is used as the final report for ATCT’s Preliminary Report and the Incident is closed.
An Unsatisfactory Response or No Response from the airport operator may result in further action being taken by the FAA.

Airport Condition Reports (NOTAM)
If assistance is required, the airport may be referred to the FAA Runway Safety Office or agencies for the appropriate guidance or training.
