2 Additional Admin Notes
You will need to be dressed in PROFESSIONAL ATTIRE for the board. Do a google search if you are still unsure what this means. Clear your schedule for the day of the board. Memoranda for excusal from classes can be requested from Cadre. Have access to your cell phone at all times. Appointment times may change at a moment’s notice. Be flexible. You will need to be at least 15 minutes early for your scheduled appointment. Appointments can last up to 30 minutes. You should be waiting on the board members, not the board members waiting on you. Showing up late will be a very bad mark against you. Prior to entering the board room, your cell phone should be placed in the OFF position, or left outside the room. You can survive 30 minutes without your phone in your pocket. If you have any questions, check with your Cadet Chain of Command (CoC). Physical address in the Phoenix Metro area is required. Cadets that live on GCU campus should provide their Building and room number.

14 Each Cadet will provide a requested location to complete their DODMERB physical.

15 Each Cadet will complete a SAL Spreadsheet.
See notes for completion instructions. SAL Spreadsheet completion instructions: This form will be typed, not hand written. The information on the template is for your reference. You will need to remove "Joe Snuffy's" information and add in your specific information. Block 1 at the top of the page should be your last name followed by your first name (example: Snuffy, Joe; not Joe Bob Jim Doug Snuffy, III). For each activity, under each school year, add the appropriate number score, total the scores across and down then explain what each score represents. Example: Mr. Snuffy, during his 11th grade year in high school, was the captain and lettered in his All-District Varsity Track team, therefore he added "2" and "1" under 11th.  As he was a only member of his Cross Country team during his 12th grade year, he added "1". He then totaled this up to the far right for “4".  After Mr. Snuffy completed all applicable lines, he added all of the specific scores for a Grand total of 17. Finally, Mr. Snuffy, in simplest terms possible, explained at the bottom of the form what each score related to. If you need more room to explain, please feel free to add lines into the spreadsheet, however, do not go into detail as to what you are trying to explain. I do not need to know what the Edge/Young club is. If the board members want to know this information, they will ask at your interview.
