5 Casualty Feeder Card (Front)
ROSTER NUMBER LAST, FIRST M. SSN AGE GENDER RACE HEIGHT (inches) WEIGHT UNIT SPONSOR NAME/CONTACT INFORMATION The casualty feeder card will be white in color and the information will be written on the side with no lines (blank). The casualty feeder card will be stored in a separate Ziploc bag in the left shoulder sleeve pocket of the ACU top at all times while on school grounds. Information will be written in BLACK marker for TEAM 1, GREEN marker for TEAM 2, RED marker for TEAM 3. This card will only be used by medical personnel in the event of a medical emergency.

6 Casualty Feeder Card (Back)
Food/ Water consumption/ Rest Day of training/event Night Prior Morning of Amount of rest Student will use the side with lines to annotate everything that the SM has had to eat or drink from the time they leave school grounds in the afternoon, until the time the student comes back to first formation the following day. Student will be required to complete a new Casualty Feeder card before the first formation of every school day.
