2 What is TeleMOVE? TeleMOVE! has evolved from the National Home TeleHealth Program (Previously known as CCHT) Home Based Weight Management Program that VAMHCS implemented in the Fall 2010 Currently staff two Registered Dietitian Care Coordinators

3 Vision & Initial Goals of TeleMOVE!
Vision of TeleMOVE!: Empower Veterans to self manage their weight from home with the support of an interactive Disease Management curriculum and a TeleMOVE! Coordinator (Registered Dietitian)

4 80% of Veterans are overweight or obese!
2015 VA Report "The U.S. veteran obesity rate has reached 80 percent, surpassing that of the general population. More than 165,000 veterans who use its health care services have a body mass index higher than 40, a threshold that interferes with basic physical functions and leads to chronic illness."

5 Who does TeleMOVE! Target?
Working Veterans and those unable to attend onsite MOVE groups Veterans with geographic or transportation challenges Eligibility requirement to pursue weight loss pharmacotherapy and/or Bariatric Surgery Veterans uncomfortable in group settings

6 TeleMOVE! Program Requirements
Cell phone or landline 8th grade reading level Commit to DAILY weighing and answering educational phone call for 12 weeks Consult from healthcare provider Stable blood pressure HgbA1c < 9.0 BMI (Body Mass Index) > 25.0 with co-morbidities or BMI > 30

7 TeleMOVE! Program 12 week cycle program cycle.
Can re-enroll for additional cycles if compliance requirements are met and making progress towards healthy lifestyle goals. Six months of participation is encouraged for behavior change. Majority of participants utilize Interactive Voice Response (IVR) phone system to listen to daily program messaging and transmit weight.

8 FY 2017 TeleMOVE! Consults 594 HT Wt Mgmnt (TeleMOVE!) consults received Not all consults were enrolled for various reasons: Did not meet eligibility requirements and/or were forwarded to Telehealth for Chronic Care Management Declined participation or No Show for scheduled appointment Staff unable to reach or contact veteran. No return contact received from veteran.

9 FY ‘17 TeleMOVE! Results 266 veterans were newly enrolled
42% female participants ~20-80 years of age 386 total veterans participated (either enrolled or graduated from program during FY ‘17 timeframe)

11 TeleMOVE! FY ‘17 Weight Management Data
TOTAL POUNDS LOST (this represents ALL participants including non-responders & those who dis-enrolled prior to 12 week cycle completion) participants lost a total of = 2694 pounds Average weight loss per veteran= 6.9 pounds The 266 newly enrolled veterans lost a total of = 1,594lbs (many of these veterans remain in the program and continue to work towards further weight loss) 52% of total veterans who enrolled lost 5 or more pounds. 38% lost 3% or more of their body weight These numbers represent ALL veterans who enrolled in the program in FY’16 including those who did not fully participate after initial enrollment. Non-responders and those who dis-enrolled prior to the 12 week program completion. If these groups were excluded, the % of veterans who fully participated in the program that lost >6 pounds would be considerably higher.
