AccidentAdvanceSM Base Benefit Modules
Accident Emergency Treatment (Module #1) 1 to 12 units Accident Emergency Treatment Benefit: Pays up to $300 or $25 per unit for treatment by a physician and X-rays received in a hospital or a physician’s office received within 96 hours of the accident. Major Diagnostic Examination Benefit: Pays up to $480 or $40 per unit for one CT Scan, MRI, or EEG. Must be completed within 90 days of the accident. Dislocation Benefit: Pays a scheduled amount based on the joint dislocated and the treatment procedure; open or closed reduction. Pays this amount for dislocations reduced under general anesthesia. A dislocation reduced without general anesthesia is limited to 25% of the benefit amount. Benefits are payable only for the first dislocation of a joint. If multiple dislocations are reduced, we will pay 1½ times the highest benefit amount and no other benefit amount will be paid under this benefit. Lowest benefits range from $30 to $800 and highest benefits range are $360 to $9,600.

Accident Emergency Treatment (Module #1) 1 to 12 units continued Fracture Benefit: Pays a scheduled amount based on the fracture sustained and the treatment procedure; open or closed reduction. A chip fracture is limited to 10% of the benefit amount of the fracture involved. If multiple dislocations are repaired, we will pay 1 ½ times the highest benefit amount and no other benefit amount will be paid under this benefit. Lowest benefits range from $70 to $670 and highest benefits range are $840 to $12,000.

Follow-Up Visits and Physical Therapy (Module #2) 1 to 12 units Accident Follow-Up Treatment Benefit: Pays up to $120 per visit or $10 per unit for follow-up treatment visits. Maximum of 3 follow-up visits per accident up. Original treatment must have been received within 96 hours of the accident. Treatment must begin within 30 days of, and completed within the 6 month period. Physical Therapy Benefit: Pays up to $120 or $10 per unit for physical therapy treatments performed by a licensed Physical Therapist under the advice of a physician. Maximum of 10 treatments per accident. Treatment must begin within 120 days of the accident and be completed within 1 year of the accident.

Initial Accident Hospitalization (Module #3) 1 to 12 units Initial Accident Hospitalization Benefit: Pays up to $3600 or $300 per unit selected once for the first hospital admission due to an accident. Benefit is payable once for the first Intensive Care Unit admission due to an accident. The ICU benefit is paid even if admitted to the hospital initially and then transferred to ICU later during the same hospitalization. Ambulance Benefit: Pays up to $720 or $60 per unit for ground ambulance and up to $3600 or $300 per unit for air ambulance for ambulance transportation by a licensed ambulance service to the nearest hospital for treatment within 96 hours of the accident.

Accidental Death and Dismemberment Rider ½ to 10 units Pays up to $100,000, chosen in increments of $5,000 for death resulting from and occuring within 90 days of an accident. Only 1 of the following benefits will be paid per covered accident. The Child Benefit is 50% of the benefit amount. Common Carrier Accidental Death Automobile Accidental Death Other Accidental Death Transportation of Remains: Pays up to $4000 or $400 per unit for transporting remains to a mortuary near the primary residence of the covered person if death occurs more than 200 miles from the primary residence. Child benefit is 50% of the benefit amount.

Accidental Death and Dismemberment Rider ½ to 10 units continued Additional Benefits for Accidental Death Benefits are paid to the survivors, if applicable, if an accidental death benefit is payable. Surviving Child Education Benefit Benefit ranges from $400 to $8000 per year.Child between the age of 17-21, full-time student at an accredited college, university, 2-year college, vocational or trade school within 365 days of the accidental death. This benefit will be paid for up to 4 years while the child remains a full-time student. This benefit will be paid for each eligible child. Licensed Day Care Center Benefit Benefit ranges from $400 to $8000 per year.Child between newborn and 12 and attending a licensed day care, who is not an immediate family member, within 90 days of the accidental death. The day care must be necessary for the survivor to work or obtain training for work. Benefits range from $150 to $3000. Career Enhancement Benefit Benefit ranges form $400-$ Survivor must be a full-time student at a professional or trade training program from an accredited college, university, 2-year college, vocational, or trade school within 24 months of the accidental death. This benefit will be paid up to 4 years while the survivor remains a full-time student..

Accidental Death and Dismemberment Rider ½ to 10 units continued Accidental Dismemberment Benefits Scheduled benefits for lowest units range from $250 to $5000 and for highest units from $5000 to $100,000. Dismemberment must occur within 90 days of the accident. If accidental death benefit is payable after dismemberment benefits have been paid for the same accident, we will deduct the dismemberment benefits paid from the accidental death benefit due. Child benefit is 50% of the benefit amount.

Accident Hospital and ICU Income Rider 1 to 15 units Accident Hospital Income Benefit Pays up to $375 or $25 per unit per day for hospital confinement for treatment of injuries beginning within 30 days of the accident. Benefit is payable for up to 365 days per accident. Accident ICU Benefit Pays up to $1125 or $75 per unit per day for ICU confinement for treatment of injuries beginning within 30 days of the accident. Benefit is payable for up to 15 days per accident.

Expanded Benefits Rider 1 to 15 units The following benefits are payable once, per person, per accident for injuries sustained in a covered accident. Burns Pays a scheduled amount based on the severity and surface area of injury sustained. There is one range of benefits chosen that is up to $1200 or up to $ Burns must be treated by a physician within 96 hours of the accident. One or more skin grafts for a covered burn will be paid at 50% of the burn benefit amount paid for the burn involved. Lacerations Pays a scheduled amount based on treatment and length of the laceration. There is one range of benefits that is chosen up to $60 or up to $900. Lacerations must be treated or repaired within 96 hours of the accident.

Expanded Benefits Rider 1 to 15 units continued Eye Injury Pays a scheduled benefit amount based on an eye injury sustained which requires surgical repair or non-surgical removal of a foreign body by a physician. Benefits paid vary from a low range of $7 to $40 and high range of $105 to $600. Emergency Dental Work Pays a scheduled benefit amount based on injury of broken teeth requiring crowns or extractions. Benefits paid vary form a low range of $8 to $30 and high range of $105 to $600. Brain Concussion Pays up to $300 or $20 per units for a brain concussion. Injury must be diagnosed by a physician within 96 hours of the accident. Coma Pays up to $22500 or $1500 per unit for unconsciousness for 14 consecutive days with no reaction to external stimuli, no reaction to internal needs, and require the use of life support systems.

Expanded Benefits Rider 1 to 15 units continued Paralysis Pays for the following categories of paralysis lasting a minimum of 30 days. Pays up to $22500 or $1500 per unit for Quadriplegia and up to $11250 or $750 per unit for Paraplegia. Tendons, Ligaments, and/or Rotator Cuffs Pays scheduled benefits based on the treatment or procedure and number of connective tissues involved. Benefits vary from a low range of $20 to $100 and high range of $300 to $ Injury must be surgically repaired by a physician within 1 year of the accident Only one benefit is payable. Major Surgery Pays up to $2250 or $150 per unit for an open abdominal, cranial, or thoracic surgery performed by a physician within 1 year of the accident. Laparoscopic procedures are excluded. Appliance Pays up to $300 or $20 per unit for a medical appliance recommended by a physician as an aid in personal locomotion. Appliances include items such as crutches, leg braces, wheelchairs, and walkers. This benefit is not payable for a prosthetic device.

Expanded Benefits Rider 1 to 15 units continued Prosthetic Devices Pays up to $1125 or $75 per unit for one prosthetic device and up to $2250 or $150 per unit for two or more prosthetic devices received within 1 year of the accident. Blood, Plasma, and Platelets Required for the treatment of injuries due to a covered accident. Immunoglobulins are not covered. Transportation Pays up to $900 or $60 per unit for up to two round trips to the hospital per covered person if special treatment and hospital confinement occurs within 30 days of the accident. The local attending physician must prescribe treatment that is not available locally. Benefits are not payable for transportation to any hospital within a 100-mile radius of the site of the accident or the residence of the covered person. Family Lodging Benefit is payable per day, maximum of 30 days, for one motel/hotel room for a member of the immediate family to accompany the covered person for treatment of injuries prescribed by a physician.

Expanded Benefits Rider 1 to 15 units continued Family Lodging Pays up to $225 or $15 per unit per day, for a maximum of 30 days, for one motel/hotel room for a member of the immediate family to accompany the covered person for treatment of injuries prescribed by a physician. Hospital confinement must be in a facility at least 100 miles form the covered person’s residence and confinement must begin within 30 days of the accident.

Wellness Benefit Rider 1 to 15 units Pays up to $150 or $10 per unit. After a 30-day waiting period, benefit is payable per calendar year for one annual health screening test for the covered employee/member and one test for a covered spouse. Health Screening Tests payable under this benefit include:

Accident Only Disability Income Rider Benefit Periods- 6 months or 12 months. Issue Ages-18 through 64. Monthly Benefits- Range from $200 to $1500 in $100 increments. Payable when a covered person suffers continuous Total Disability as a result of a covered accident, not to exceed the Benefit Period. Total Disability must occur within 90 days of the accident. This rider terminates the first of the month following the employee/member’s 70th birthday.

Sickness Only Disability Income Rider Benefit Periods- 6 months or 12 months. Issue Ages-18 through 64. Rates- One set for 18 through 49 and one for 50 through 64. Monthly Benefits- Range from $200 to $1500 in $100 increments. Payable when a covered person suffers continuous Total Disability as a result of a covered sickness, not to exceed the Benefit Period. A 14-day elimination period must be satisfied before benefits become payable. During the benefit period, benefits are not payable and do not accrue. Coverage for pre-existing conditions may be excluded or limited under this rider. This rider terminates the first of the month following the employee/member’s 70th birthday.

Spouse Off-the-Job Accident Only Disability Income Rider Benefit Periods- 6 months only. Issue Ages-18 through 64. Monthly Benefits- Range from $200 to $800 in $100 increments. Payable when a covered spouse suffers continuous Total Disability as the result of a covered off-the-job accident, not to exceed the Benefit Period. Total Disability must occur within 90 days of the accident. This rider terminates the first of the month following the employee/member’s 70th birthday.
