4 Important TRS and Enrollment Information
The plan year for Leakey ISD is September 1 - August 31 of each year. Open Enrollment for returning employees will be held July 1st until August 22, 2017. Per ACA requirements, EVERY employee is required to log into the Benefits HUB and list dependents weather you plan coverage for them of not. Please review personal information and elect or decline medical or other supplemental benefits. Remember: To make a mid-plan year change to you benefits, you must have a Qualifying Event. Examples: Marriage (including same-sex marriages), Divorce, Birth or Adoption, Death, Change in Dependent Eligibility Status, etc.

9 ActiveCare 1-HD is an HDHP and is HSA eligible.
Health Savings Account An HSA works with a High Deductible Health Plan (HDHP) and lets you set aside a portion of your paycheck – before taxes – into an account to help you pay for medical, dental, vision and prescription expenses. ActiveCare 1-HD is an HDHP and is HSA eligible. 2017 Annual Maximums: $3400 for Individual $6750 for Family Your HSA stays with you, even if you change jobs. An HSA’s balance WILL rollover every year. Only the balance in your account is available for you to use. Unlike an FSA, contributions to an HSA are NOT up-fronted to you. At 65 you can withdraw your funds to use as you like. It becomes taxable income if you withdraw. 8

AN FSA IS A TAX FREE ACCOUNT FOR YOUR OUT-OF-POCKET MEDICAL EXPENSES. 2017 Annual Maximum: $2600 It is easy to use--just swipe it as a payment for your eligible medical, dental, vision and, prescriptions expenses) The expenses are automatically deducted from your FSA balance. All participants of FSA receive a flex card at NO COST! Employees will receive 1 NBS flex cards You will receive your new NBS debit card mid-September. DEPENDENT CARE FSA IS A TAX FREE ACCOUNT FOR YOUR CHILDCARE EXPENSES. Annual Maximum - $5,000 for married couple filing jointly or $2,500 if filing single.

11 MEDLink® Plan by APL MEDlink is designed to supplement your employer’s medical plan. This plan provides supplemental coverage to help offset out-of-pocket costs that you may experience due to deductibles and coinsurance of your employer’s medical plan. In-Hospital Benefit Pays expenses you incur as an Inpatient (at least 18 continuous hours) $1500 or $2,500 per confinement Out-patient Benefit Pays up to $ per treatment in: Outpatient surgery in a Outpatient Facility Outpatient surgery in a free-standing Outpatient Surgical Center Diagnostic testing in a Hospital Outpatient Facility Diagnostic testing in a MRI Facility Physician Outpatient Treatment $125 per family per year All benefits for the same or related conditions will be subject to the maximum benefit, unless such conditions are separated by 90 consecutive days, then a new maximum out-patient benefit will apply. You are NOT eligible if: Covered by TRS-Care, Medicare, Medicaid or Medical Savings Accounts. Have an actively funded HSA Employees (or their dependents) who are not covered under the school’s major medical plan. Non-residents of the USA. Employees not actively at work on 9/1

Dental by Cigna Increase for Plan year High Plan covers each person for $1500 per year & $50 deductible per person. Staying in network gives you more bang for your buck! Low Plan covers each person for $750 per year & $50 deductible per person. No out of Network on Low Plan Preventive Benefits covered at 100% in both plans DHMO Plan is a Discount Plan and you must choose a Network Provider Vision by Superior No Changes for Plan year $10 Copay for exam and $25 Copay for materials up to $125 & $120 for Contact Lenses $25 fitting fee — Discounts Features for certain add-ons. See a In Network Provider to stretch your coverage dollars

Additional Supplemental Benefits Disability with Unum Protection for you pay check up to 66 2/3 Elimination periods from 7 to 180 days In-Hospital Benefit Waiver for 7/14/30 day Waiting Periods Pre-existing applies if medical condition is present 3 months prior to Sept 1 Life Insurance with Cigna Basic Life $20000 paid by LEAKEY ISD and you must designate a beneficiary Guarantee Issue for new employees for additional Life Insurance up to 200k for employee - 50K for spouse and 10k for Children Current enrollees can increase coverage but must submit Evidence of Insurability Cancer Plan with American Public Life No Changes for Plan year Guaranteed Issue; 12 month waiting period for any pre-existing conditions Helps offset costs not typically covered by Medical Insurance such as Radiation and Chemotherapy, Lodging, Transportation, Experimental Treatments, second and third opinions and much more….
