What’s the purpose of Section 5310? To improve mobility for seniors and individuals with disabilities by removing barriers to transportation services and expanding transportation mobility options;

What’s my role as a Human Service Provider? To provide safe and efficient transportation based on the approved project scope and to coordinate services when feasible; What are my responsibilities as a Subrecipient? To follow all criteria outlined in the contractual Sub-grant Agreement and the OPT State Management Plan;

Are you familiar with those documents? The Sub-grant agreement is your contract The OPT State Management Plan is located on our website and it provides guidance to all areas of OPT. What happens after the SCDOT Commission approves my project? The agency receives a request for Certification & Assurances from the OPT Grants Section and the OPT/FTA Grant process begins…..

How do I know my Contract/Sub-grant Agreement is fully executed? Once your agency signs the agreement and returns to OPT you will receive a letter stating the agreement is fully executed. You should not proceed until you receive this document. If you are not sure, contact your Regional Program Manager or HS Program Manager for assistance. If you proceed without approval, you are assuming the financial responsibility of paying 100% of your project.

These are the main HS focus areas this fiscal year . . Program Management (Scope & Project Success) Annual Reporting (2 Forms) Program Compliance (HS Questionnaire/Triennial) Vehicle Maintenance (Vehicle Maintenance Plan) (Required to comply with all above as long as we are lienholders)

8 Human Service Reporting
Please make sure you use the updated form (see OPT website) Proof of Insurance (provide a copy) Mobility Mgt. must complete Performance Section including number of clients served; Everyone completes the Project Summary on page 3

9 Human Service Reporting
1 HS Program Measures Report required per project and/or vehicles with lien; Due 10/18/2017 Capture ridership through 9/30/17; Individuals eligible please report the overall amt. for the counties served, (you can estimate total).

10 Human Service Reporting
All agencies are required to have a Compliance & Oversight Questionnaire on file. The form will remain on file for 3 years. If changes occur within the 3 year review period, please send in an updated Questionnaire, especially your Executive Members.
