Provides employees and their families with financial security in the event of the death of the insured The Employee enrolls during their initial enrollment period (when hired) or an open enrollment. If application is for an amount under the Guarantee Issue Amount (GI)* there are no medical questions or physical exam. Companies can choose to also offer coverage for employee’s family members (spouse and children). Pays a flat benefit amount to the beneficiary when the member dies. * Guarantee Issue Amount is the amount of coverage a person may receive without submitting Evidence of Insurability**. ** Evidence of Insurability (EOI) is proof of a person's medical history. LifeMap uses the EOI to determine if the person can be approved for insurance.

Benefits Includes a Beneficiary Assistance Program; provides counseling, financial help and more to the beneficiary when a member dies. Terminally ill employees, with a shortened life expectancy, may be able to access a portion of their life benefits right away. An Accidental Death and Dismemberment (AD&D) benefit is available with this product

Helps make up for lost income in the event of disability Features Two kinds of disability insurance: Short Term (STD) and Long Term (LTD) STD is typically up to 26 weeks LTD is typically up to age 65 or Social Security Normal Retirement Age (SSNRA) Benefit payments are made weekly for STD, monthly for LTD Available as Basic (employer-paid) or Voluntary (employee has premium deducted from their paycheck) Benefits The benefit is either a flat amount or a percentage of pay Covers disabilities from illness (including pregnancy) or injuries If a disabling illness or injury prevents an employee from performing their current job, LTD insurance can include assistance to find a suitable occupation (and/or training) for the employee

Helps make up for lost income in the event of disability It eases the burden on employees and their family members if they can no longer work Nearly 18.5% of Americans are currently living with a disability, and 1 out of every 4 persons in the US workforce will suffer a disabling injury before retirement Accidents are NOT usually the culprit. Back injuries, cancer, heart disease and other illnesses cause the majority of long-term absences. Workers’ Compensation only covers an employee if they get injured or ill on the job. Fewer than 5% of disabling accidents and illnesses are work-related

Provides employees and their families with financial security and peace of mind in the event of death by accident or loss of some functions Employee enrolls during their initial enrollment period (when hired) or an open enrollment There are no medical questions or a physical exam needed Basic AD&D only covers the employee Companies can choose to also offer coverage for employee’s family (spouse and children) on Voluntary Pays a benefit to the beneficiary if the member dies due to an accident If a member survives an accident but loses sight or a limb, or becomes paralyzed, the employee may receive a percentage of the AD&D benefit based on the severity of the loss. AD&D is usually sold as part of the life policy; but, it can also be sold as a stand-alone product

Places dental care back in hands of the patient and the provider. Very few exclusions (including cosmetic and orthodontic) No age restrictions and pairs well with Medicare Available in Oregon, Washington, Idaho, and Utah.

Offered as a PPO* or Open Access Plan** Dollar-Based Dental: Pays benefits based on a % of a dollar amount. Offered as a PPO* or Open Access Plan** Managed Care Dental: Dental HMO, Offered through a partnership with Willamette Dental Group (WDG), only available in OR *PPO is a plan that utilizes a network, allowed amounts are based on the negotiated amount for each service ** Open Access is a plan that does not utilize a network, allowed amounts are based on the usual and customary fee for that service

Short Term Medical Insurance (STM) is designed to provide medical coverage on a temporary basis to fill a gap when a person does not have medical insurance STM cannot be renewed and is not intended to replace permanent coverage The person applying for coverage chooses the length of policy they would like, from 30 – 185 days They then choose the deductible amount they would like, from $500 - $7,500 per member and the coinsurance level they would like to pay, either 20% or 50% STM provides coverage for injuries and sudden-onset illnesses which occur while the policy is in effect

There are two Employee Assistance Program (EAP) options available; Basic and Enhanced The Basic EAP is automatically included with all LTD plans and an employer can buy-up to the enhanced plan An employer can purchase either the Basic and Enhanced EAP plans as “stand-alone” when any other LifeMap product is purchased The employer pays for 100% of the cost of this inexpensive benefit Both the Basic and Enhanced EAP offer life-balance services and resources for employees and their families including counseling, legal direction, financial advice and more
