(This includes services and generic prescriptions.) Benefits for Preventive Care Services as required by law are covered at 100%. 11/7/2018 Network Providers To find a network provider/clinic near you go to: Select UHC options PPO Enter Search Criteria Select Specialty View Results If you utilize a UHC Network provider, you will pay a $50.00 deductible, insurance will pay 80% of the covered medical expense and you will pay the remaining 20%. The deductible is the first dollar amount applied to the bill that you are responsible for before the insurance begins to pay for the covered services. There is unlimited Annual and Lifetime Dollar Limits on Essential Health Benefits. 11/7/2018 Prescription Coverage Boynton Health Services: - $10.00 copay for generic prescriptions - $15.00 copay for all other prescriptions UnitedHealthcare Network pharmacies - $15.00 copay for generic prescriptions - $30.00 copay for Tier 2 prescriptions - 40% co-insurance for Tier 3 prescription No Coverage for Out of Network pharmacies If available, most medical providers try to prescribe generic prescriptions for their patients. 11/7/2018 Student Insurance Student Insurance

7 Understanding the Process
11/7/2018 Understanding the Process Use the Emergency Room ONLY for life threatening illness/injury or potential for permanent disability. NurseLine (Collegiate Assistance Program) Carry your insurance card with you at all times. The Nurseline is available 24/7 to answer any questions or help you determine what the best plan of care is for you at the time. 8 Insurance Requirement
11/7/2018 Insurance Requirement All F-1/J-1 International Students Exception – students with a sponsoring agency or government 2) A Refund is available if you qualify The reason this is a requirement is that the MN Colleges and Universities must guarantee that students have everything they need to get an education: that being food, lodging and healthcare. If anything medical were to happen it would put a huge financial strain on students, which may prevent them from completing their education. All refund requests must be approved and the form signed by the International Student Advisors. 11/7/2018 Examples of Covered Services Under Preventive Health Care – wellness physicals are covered; many immunizations such as MMR, Tdap, flu vaccine STD testing: Gonorrhea, Chlamydia & HIV testing Acne treatment and medication One vision exam per year, up to $100 This policy covers many medical services but here are a few examples of covered services that students frequently inquire about. 10 Examples of Non-covered Services
11/7/2018 Examples of Non-covered Services Eye glasses (frames, contacts) Over the counter drugs that do not require a prescription Routine foot care Dental (unless injury to a natural tooth) All medical policies have exclusions and here are a few common examples of non-covered services under this policy. 11 11/7/2018 Online Access If you have an account, log in to My Account – If you do not have an account, click on “Create My Account Now” link. United Healthcare no longer automatically sends you your insurance card. Once they receive your insurance payment and have your application information in their system, they will send you an advising you to log in to “My Account” or “Create My Account Now”. Once you have created your account, you will be able to print your own insurance card, view your claims, update your address and many other options.
