United State Department of Agriculture Navigating the Verification Process A Diagram for Households How to use this resource The slides in this PowerPoint illustrate the verification process from the perspective of a household. Visual aids often enhance understanding over plain written instructions and may be helpful to the families in your district. This resource can be printed and sent out as a flier with other verification materials, or posted on your website as supplemental information. The first diagram (slide 2) “leads” parents to the action items that may apply to them by asking a series of yes/no questions. The second diagram (slide 3) shows some of the ways that households can submit verification documents to the district. The next slide provides tips on how to customize these resources to fit the needs of your district.

2 Customization Before making these resources available to families, be sure to customize them with your district’s information, where appropriate. On the “How to respond to your verification request” diagram: Update the names of the assistance programs as they are known in your State Insert the contact information for your homeless, migrant, runaway coordinator(s) On the “Send information in any of these ways!” diagram: Insert your district’s name, phone number, and mailing address for verification responses Make sure all the ways to respond are consistent with your districts’ policies and practices Be sure to remove all of the “<< >>” throughout – they are only intended to highlight the fields that needed to be customized! Customize the flier in other ways to meet the needs of families in your district! Note: for 508 compliance, the objects are currently “grouped.” To make modifying your timeline easier, “ungroup” the objects by right-clicking on the timeline graphic and selecting “ungroup” from the “Group” drop-down menu. 3 How to respond to your verification request
Contact <<school, homeless liaison, or migrant coordinator>> at <<(xxx)-xxx-xxxx>> or << >> for help. Question 1: Do you or someone in your household receive <<SNAP>>, <<TANF>>, or FDPIR now or at any time since you applied? Question 2: Is your child homeless, migrant or runaway? No Yes Question 3: Is your child a foster child? Please send us your <<SNAP>>, <<TANF>> or FDPIR Certification Notice that shows dates of certification. Do not send your EBT card. If you send us one of the above documents, you are DONE. You do NOT need to send anything else. Send documentation that verifies the child is the legal responsibility of the agency or court. OR, provide the name and contact information for a person at the agency or court who can verify the child’s foster status. Send documentation for either the month before you applied for school meal benefits, or any month since. The document(s) must show: Name of person who received the income Date received Amount received How often it was received Acceptable Documents for Showing Household Income Jobs: Paycheck stub or pay envelope that shows the amount and how often pay is received; letter from employer stating gross wages and how often you are paid; or, if you work for yourself, business or farming papers, such as ledger or tax books. Social Security, Pensions, or Retirement: Social Security retirement benefit letter, statement of benefits received, or pension award notice. Unemployment, Disability, or Worker’s Compensation: Notice of eligibility from State employment security office, check stub, or letter from the Worker’s Compensation office. Welfare Payments: Benefit letter from the <<State TANF>> office. Child Support or Alimony: Court decree, agreement, or copies of checks received. All Other Income (Such as Rental Income): Information that shows the amount of the income, name of the person who received the income, the date it was received, and how often it was received. Military Housing Privatization Initiative: Letter or rental contract showing that your housing is part of the Military Privatized Housing Initiative. If you do not have income, please send a brief note explaining how you provide food, clothing, and housing for your household, and when you expect an income. 4 Send information in any of these ways!
