Major changes Changes in Active Care 1 HD Comparison of Active Care 1HD and Active Care 1 Changes in Active Care 2 Changes in Active Care 3 Our monthly premium and comparison of prior year and current year rates

Eliminated Active Care 1 for Individual and Family. Active Care 1 qualifies as a high deductible plan for employee only but not for family coverage Active Care 1 HD qualifies as a high deductible for all tiers Those in Active Care 1 will automatically be enrolled to Active Care 1 HD effective September 1, 2013 If employee does not want Active Care 1 HD, the employee may choose to enroll in Active Care 2 or Active Care 3 No changes in Active Care 3 because benefit reductions would cause Active Care 3 to no longer be in compliance with law requiring it to be comparable to Health Select

Difference between Active Care 1 and Active Care 1 HD Deductible per plan year for individual increased by $1,200 Deductible per plan year for Family increased by $1,800 Out-of-pocket maximum per plan year for individual increased by $1,850 Out-of-pocket maximum per plan year for family decrease by $1,800 Premium rates in 1 HD are 9%-22.7% higher than 1 Premium amounts listed is employer and employee amount

ACTIVECARE 2-Changes for 2013-2014 Deductible per plan year for individual increase to $250 Deductible per plan year for Family increase to $750 Out-of-Pocket maximum per plan year for individual increase to $2,000 Out-of-Pocket maximum per plan year for family increase to $2,000 15% increase of premium in all tiers Premium amounts listed is employer and employee amount $5 increase in copays for 31 day supply of drug purchased at retail, (from 30 day supply)
