3 Suspending usage of Large Flat Rate Boxes
Effective immediately, Urinalysis samples will no longer be mailed utilizing the Priority Mail Large Flat Rate Boxes. All unused LFRB can be returned to the servicing Military Post Office and/or Unit Mail Call.

4 Preparation of Samples
The IOMC, MCB Camp Pendleton, will only accept urine in individual boxes. 12 bottle samples boxes 6 bottle sample boxes Single sample boxes Other Do not bring urinalysis boxes already consolidated into larger boxes.

5 Preparation of Samples for Mailing
All samples must be individually wrapped in approved bags to prevent spillage. Do not put more than one box inside each bag. The Post Office will only check to ensure all boxes are properly sealed for mailing, per USPS regulations.

The purpose of the leak proof secondary container is to prevent leaks. The secondary container must be properly sealed. Staples should not be used to seal bags. Tape must cover the entire opening of the bag.

9 Mailing Urine Samples Bring all properly sealed samples to your local Military Post Office/Unit Mail Call for consolidation and mailing. The Local MPO/UMC will assist with the consolidation of samples. We will no longer provide USPS boxes to SACOs. We will not accept already consolidated sample boxes.

10 Mailing Urine Samples Each SACO must bring properly addressed To: and From: labels to affix to the consolidated box. Addressing Standards per DOD M and MCO A Return and delivery addresses will be typed No hand written or stamped addresses

11 Mailing Urine Samples Final boxes must contain the following labels:
Clinical specimen label Biohazard Label It is the SACO’s responsibility to provide mailing labels and biohazard stickers. Correct and proper packaging and addressing will prevent any delays and make it a smooth mailing process.

12 Mailing Urine Samples Each sample box being consolidated does not need to be individually addressed or prepared with a biohazard and a clinical sample label. Only the consolidated box needs to have: To: Label From: Label Biohazard Sticker Clinical Sample Label It is the SACO’s responsibility to provide mailing labels and biohazard stickers. Correct and proper packaging and addressing will prevent any delays and make it a smooth mailing process.

13 Mailing Urine Boxes are consolidated in batches of four.
Each sample box not being consolidated, must be individually prepared for shipment.

14 Urinalysis Mailing Process (Multiple Sample Boxes)
Once urinalysis is complete: Prepare and seal each sample box. Ensure each box is properly sealed in a secondary leak proof container. Bring samples to MPO/UMC Do not consolidate samples into a larger box. Ensure you have required labels: To, From, Biohazard, and Clinical Sample labels with you. A Postal Marine will help consolidate samples by batches of four. SACO will provide proper labels Labels will be affixed only to the consolidated box. Left over sample boxes are prepare individually for shipment.
