Introduction to Services

2 Do Now: On a piece of scratch paper, take 45 seconds to write down up to 5 reasons why someone would visit a clinic. As students get settled, ask them to turn their attention to the Do Now. Ask them to reflect on the question silently.

3 Purpose Many students need health services, but (for many reasons) do not get them. The School-Based Clinic aims to provide easy access to high-quality health services so that you can get your basic needs met and focus on living a healthy life.

5 What is the School-Based Clinic?
It’s a clinic… High-quality, affordable, and comprehensive medical and behavioral health services by professional staff who specialize in adolescent health. It’s in your school… Located in room E021 Open Monday – Friday, 8:30AM – 3:00PM Open to all students Facilitator: Make sure you introduce yourself and explain your role at the clinic.

6 What Does the Clinic Do? We provide services related to:
Physical health Mental health Nutrition Safe sex information STI testing and treatment Birth control & pregnancy testing Help getting health insurance Physical health: Physical exams/checkups (for sports, college, jobs, etc.), immunizations, treatments for common conditions (like the flu, cold, headaches, cramps, etc.) Mental health: individual counseling, connecting to community resources Nutrition: healthy eating habits, how to gain/lose weight in a safe way Safe sex education: STI testing and treatment, information about sexual practices Reproductive health: birth control, pregnancy tests

In groups of three, share one reason why you think the clinic provides these services to high school students: Physical health Mental health Nutrition Safe sex information STI testing and treatment Birth control & pregnancy testing Help getting health insurance Facilitator: Give the students (example: 150 seconds [2 ½ minutes]) to discuss in a small group why these services are important to high school students. As they discuss, walk around the room to listen in on conversations; select 2-3 groups to call on when you convene. Some possible answers could be: Easy access (it may be hard to get to doctor due to transportation issues, time constraints, where they live, etc.) Being able to talk to someone in a safe, judgment-free environment (without parents) Get accurate information about a variety of topics – knowing is better than not knowing! To get support and help if something comes up in the middle of the school day Personal health and academic success are linked; if you aren’t a healthy individual, you aren’t going to be able to thrive in the classroom. Be prepared to share out to the rest of the group!

8 Our services are… Discreet Convenient Affordable
Discreet. We offer (mostly) confidential services at the clinic. While we are housed at the school, we operate more like a doctor’s office in terms of protecting your privacy. However, in most cases, if you are under 18-years old, there are things that we legally need permission from your parents/guardian(s) for before we can see you (for example, mental health services or physicals). Other things, like reproductive health or chemical use treatment, are things for which you can give consent. We do everything we can to maintain your privacy, like not talking to teachers about what you come to the clinic for and not sharing details about our time together with your parents (unless you say it’s okay). Convenient. We’re located right in the school. No need to worry about transportation or missing lots of class time. Affordable. Everyone is covered, regardless of insurance or ability to pay. If you have insurance, we will bill it, but you will never have any out-of-pocket costs.

11 How to Register Registration & Consent Form (GREEN)
Sent out with enrollment forms. Must be signed by parent/guardian if under 18 years old Consent for most treatments at the clinic (exception: mental health) Minor Registration & Consent (PINK)  Signed by the student Gives consent to reproductive health & drug/alcohol abuse Green registration & consent form Must be completed and signed by your parent/guardian if you are under 18 and returned to the clinic – once we have this form, it is good for all 4 years! Minor’s Consent Minor’s consent is an exception to the rule that your parents have to agree before you can receive health services if you are under 18. It only applies to some services though. At the clinic, this means you can complete your own pink form and be eligible to receive certain services such as those related to reproductive health (like birth control, condoms, pregnancy testing) and drug/alcohol abuse. Confused? Stop by the clinic.

12 Mental Health Informed Consent  Must be signed by parent/guardian if under 18 years old Must be (re)signed after 12 months

GREEN: Clinic Registration/Consent Must be signed by parent/guardian (if student is under 18) Gives consent to most services provided in the clinic. Good for all four years of high school RED: Minor Consent Can be signed by a student under 18 years old (parents do not have to sign it) Gives consent for reproductive health (birth control, sexual education, STI/pregnancy testing) and drug/alcohol abuse WHITE: Mental Health Must be signed by parent/guardian (if student is under 18) before receiving mental health services. Gives students permission to be seen by a mental health professional Must be (re)signed each year until client turns 18 yrs. SIMILARITIES: Must be signed and turned in before a student can receive services. Extras can be found in the clinic

14 Visiting the Clinic Submit all registration forms.
Call OR or stop by the clinic (E021) Talk with Atasha at the front desk to pick a day & time for your appointment. Write down your appointment time. Put it on your calendar, make a reminder on your phone, or keep the confirmation card. Your teacher will get a pass from us that excuses you from class the day of your appointment. Come to the clinic and check in with Atasha. Your provider will greet you and you can get started with your appointment! Check out with Atasha and schedule a follow-up appointment, if needed. 1. If you need a new form, we have extras at the clinic. 2. Students can stop by at the following times: before school, during lunch, or passing time. Please check in with your teacher and get a pass first! 3. We can schedule an appointment on a day/time that best fits your schedule. Please be aware that you might have to wait a few days for an open appointment. 4. Be aware that such reminders may not maintain your confidentiality. Don’t keep your card hung up in your locker. Make a code word in your phone. 5. Please go to class FIRST and get your pass from your teacher.

15 When you are done, turn your boards around.
Closure On your whiteboard, write down one thing that you: Learned about the Southwest School Based Clinic OR One thing you are still curious about. When you are done, turn your boards around. Facilitator: Encourage students to silently read the boards of those around them. As you glance around the room, answer questions that students have identified and/or address any misinformation.
