5. Clinical Documents Immunizations, CPR, and physical
Important deadlines are on the front of the physical form
Faculty will review. However, you will upload the documents to Castlebranch
Castlebranch will review and maintain all required clinical documents

6. CPR
Current American Heart Association Basic Life Support for Healthcare Providers (CPR) card (no other providers will be accepted); this course must include the following components:
Adult, infant, child, one-man & two-man rescue, choking, use of mask/artificial airway/automatic external defibrillator (AED).
Strictly online CPR courses are not acceptable
Must have a face to face component
Please don’t forget to sign the back of your card

We will use this photo ID as your nursing program ID
You will need a new badge that states “nursing student”.
