1 Welcome to the 2018-2019 UIW Annual Benefits Open Enrollment

2 Open Enrollment Period April 25 – May 9, 2018
Open Enrollment is the time to sign-up for medical, dental, vision or flexible spending Enroll or waive benefits online through Banner Self-Service via MyWord Portal. Benefits will not roll over from the previous year. Benefit elections are effective June 1, 2018.

3 Benefit Changes Gilsbar - third-party administrator for Medical, Pharmacy, Dental. UIW Bronze and Silver plans have an open provider option with reference-based pricing.    The Gold plan will no longer be offered and will end on May 31, 2018.  Cigna Silver Plan – network of providers and same coverage as the UIW Silver plan. Dental coverage - an open provider option. MaxorPlus - Pharmacy benefit. Health care flexible spending will increase from $2,600 to $2,650.

4 What’s staying the same
UIW Bronze and Silver plan premiums and coverage will remain the same. Deductibles and out of pocket maximums will remain the same. The annual health fund will continue for the UIW Silver and Cigna Silver plan. Dental plan premiums and coverage will remain the same. MetLife will continue as Vision coverage and premiums will remain the same. Medical subsides will continue for those earning below $48,000. SBS Flex Spending, Aflac, and Catholic Life with continue. 

5 Working Sessions The benefit enrollment working sessions are online step by step guidance in a computer lab. All working sessions will be held in the Mabee Library computer Lab, room 230.

10 Flexible Spending The FSA allows the participant to open up to two types of special “Spending” accounts. Medical Reimbursement Account Allows employee to pay for their families unreimbursed, qualified health (medical, dental, vision, Rx) related expenses using money set aside from their paycheck on a pre-tax dollars The maximum annual election amount is $2,650 Dependent Daycare Account Allows the employee to pay for dependent daycare expenses The maximum annual election amount is $5,000 if single or married filing joint tax returns; $2,550 if married filing individual tax returns

11 How Does The FSA Work? Estimate your families unreimbursed health/daycare expenses for the upcoming plan year Enroll in the FSA Use your debit card to pay for eligible expenses as they are incurred; file manual claims for daycare reimbursement Eligible Expenses Co-Payments, Deductibles, Coinsurance Dental Expenses (Fillings, Root Canals, Crowns, Bridges, Extractions, Orthodontia) Glasses, Contact Lenses, Contact Lens Solution Prescription Medications OTC – Allergy Meds, Cough Syrup, Band-Aids, Rubbing Alcohol, Sunscreen, First Aid kits

12 Important Rules Receipts – Be sure to keep copies of all receipts; including those for services paid for with the Flex Convenience Card Use or Lose Rule – any amount that is left in the account at the end of the plan year that is unclaimed is forfeited. Plan carefully and be conservative in your estimates. Grace Period through 8/15/19. Timely Filing Deadline – the deadline for filing all health claims is 90 days after the end of the plan year, (11/15/19) or 90 days from your termination date; the deadline for daycare claims is 8/31/19 – Grace Period does not apply Flex Card Transactions – some transactions will require that you send a copy of your receipt after your purchase. SBS will notify you when it’s time to send a copy.

17 Aflac AFLAC supplemental policies pay CASH benefits directly to you in case of accident or illness. Each policy is voluntary and all benefits are payable directly to you. You decide how to use these benefits: replace lost income, take care of everyday living expenses, deductibles, co-pays and out of network charges, and travel expenses are just a few of the ways these benefits can help you.

18 Short Term Disability Pays a monthly benefit if you are unable to perform the (rates quoted individually) duties of your job due to an off the job accident or illness. Each employee chooses the monthly benefit amount and the elimination period. These benefits come directly to you and you decide how to use them. Accident Plan Pays benefits for injuries and their treatment directly to you in case of accident—the initial treatment, the injuries, follow up care, physical therapy, major diagnostic exams, hospital and intensive care stays, ambulance transportation, a death benefit and a wellness benefit. It is not necessary that you are unable to come to work for benefits to be paid. On and off the job accidents are covered. Hospital Plan Guarantee issue hospital plan provides a lump sum benefit for hospital stays plus an emergency room benefit, PHYSICIAN BENEFIT COPAY BENEFT, as well as an ambulance benefit. The buy-up version provides a surgical benefit, complex imaging benefit, and daily hospital benefits.

19 Aflac Cancer Plan Pays a large lump sum benefit upon diagnosis of an internal cancer and then pays daily or weekly benefits for hospitalization, radiation, chemotherapy or experimental treatment, blood and plasma, extended-care, hospice and home care as well as benefits for anti-nausea drugs, immunotherapy drugs, surgery, second opinions, evaluations at the National Cancer Institutes, prosthetic devices ambulance transportation, family lodging, and bone-marrow or stem cell transplantation. An annual wellness benefit of $75 is also included. NEW: Aflac Plus Rider: Pays a $5000 benefit upon diagnosis of heart attack, stroke, type 1 Diabetes, traumatic brain injury, Alzheimer’s disease, Parkinson’s disease, ALS or Lou Gehrig’s Disease, MS and a number of other major medical events. There is a secondary event benefit, a heart surgery benefit and a tier two critical event benefit as well. This information is for illustrative purposes only. Policy provisions will prevail.
