2 CLUSTER HIGHLIGHTS One of our individuals is celebrating his 14th year with Meals on Wheels. He started as a volunteer and is now a paid employee. An individual who has never showed any interest in obtaining employment participated in a seasonal employment opportunity this year for the first time with Liberty Tax. He did not miss any days and enjoyed the experience. An individual participated and acted in the Wendy Saba Stepping Stone theater group last July 2014. An individual was diagnosed with a type 2 Diabetes in March 2014 and he turned it around with weight loss (30 lbs.) and healthy diet. He is no longer diabetic.

19 CPR ACTION PLAN ISSUE: Increase the clinical hours per client/per month COMMENTS: Review the reporting and documentation of clinical hours – ensure the clinical hours are correctly reported on MMR.
