Mrs. Amy Shubert, R.N. (Ext. 5470) Medication All medication must be delivered to the nurse in the original prescription bottle or over the counter medication packaging. All medication must be delivered to the nurse by an parent/guardian Students may not carry medication into/out of school or have on their possession at any time. (Examples of exceptions – inhaler or an EpiPen w/doctor’s note) Physician’s notes and medical issues should be communicated directly to Mrs. Madigosky 5 minutes

Medical Services Physicals for all 6th grade students are due by September 30, 2018 Tetanus, diphtheria, and pertussis (Tdap) and meningococcal conjugate vaccines are due by the first day of 7th grade Students will be excluded from school if they do not have documentation regarding these vaccines Please plan accordingly

Attendance Law & Policy
Valid excuse notes must be submitted within 3 days of the student’s return to school or the absence will remain unexcused. The following are valid reasons to be excused from school: illness, danger to health from serious exposure, quarantine, recovery from an accident, death of a family member, medical or dentist appointment when necessary, authorized school activities, pre-approved educational travel (limit to 5 days), pre-approved college visits (limit to 2 days per year), pre-approved religious instruction (limit to 36 hours per year), and observance of the student’s religion. Information will be sent home in October Coordinators Matt Crater, Trish Young, and Michelle D’Avella Dates of trip May 29th, 30th, and 31st Camp Canadensis Canadensis, PA Approximately 2 ½ hours from Havertown, PA HMS website > Academics > 6th grade > Camp Canadensis

Medical Information Medical support at camp will provide the same level of care and service that our school nurse provides Medical facility staffed 24 hours a day Accommodations for food allergies Peanut free facility

Parent Chaperones All potential chaperones must acquire the following clearances and deliver them to Ms. Essie Schiller at the Oakmont Building before February 1, 2019: PA State Police Background check FBI Criminal Clearance PA Child Abuse Clearance All potential chaperones must complete a Camp Canadensis Chaperone Application before February 1, This application can be obtained from Mrs. D’Avella The principal will select chaperones based on program need Chaperone cost, $110

Please Note… Students must commit to attend camp by April 5, 2019
If your child is expressing hesitancy about the Canadensis trip, then please contact Mrs. Hughes Discipline issues may prevent a student from attending the trip Please check your family schedules now to avoid conflicts with the camping trip

Cabins Students are guaranteed to be in a cabin with at least 1 friend. There will be 9-12 students in each cabin. Male chaperones will be assigned to a male cabin. Female chaperones will be assigned to a female cabin. There is electricity and running water in all cabins.

Activities Randomly selected groups of students and parent chaperones Examples include: boating, cooperative games, orienteering, swimming, t-shirt art, climbing “the wall,” cooking, pond study, floor hockey, bracelet art, and many other cool activities!

Meals Many selections for breakfast, lunch, and dinner
The cafeteria can honor all food allergies they are notified about The cafeteria is a peanut-free facility

Classroom visitation Start off the night by reporting to homeroom
10 minutes Classroom visits Follow “A” day schedule 10 minutes in each class The evening will end at 8:23 PM Curriculum HMS website > Academics > 8th grade > 8th grade curriculum Please avoid discussing personal/confidential/individual student information with staff tonight.
