Putting it in Writing HIMS Independent Medical Sponsor (IMS) Evaluation and Recommendations: I agree to serve as Mr. John P. Flyguy’s Independent Medical Sponsor (IMS) in his efforts to seek/maintain re-certification of his First Class Medical certificate under the HIMS-Substance Abuse Special Issuance Authorization program. It is my recommendation that Mr. Flyguy be granted a Special Issuance Authorization (SIA) of his First Class Medical certificate under the HIMS-Substance Abuse program.
