4 BRIDGE Legislation (H.B. 400) Update
Middle school students will receive counseling and advisement on career exploration, career pathways, and career interest inventories. 6th & 7th grade students will have accounts on a career information system website and complete career interest inventories. By the end of the 8th grade, MS students should have an individual graduation plan developed with consultation by the students’ counselor and parents or guardians. 5 BRIDGE Legislation (H.B. 400) Update
ALL high school students should receive annual counseling and advisement in consultation with their parents, guardians, (or designee,) and school counselor to reassess their individual graduation plans. The student’s individual graduation plan should assist with seamless transition into postsecondary, further training, or employment. BRIDGE Legislation High School Requirements

6 A Review of IDEA and Transition
completed beginning not later than the first IEP to be in effect when the student enters the ninth grade or sixteen years old or younger if appropriate by IEP Team and updated annually and include Transition Assessment and Course of Study. post secondary goals in the areas of: training/education; employment; and independent living, as measurable IEP Transition goals, activities and services that are used to reach the post secondary goals in the areas of training /education, employment, related services, community experiences, post school and daily living skills if appropriate. The student must be invited along with appropriate agency representation as needed 11/10/2018

7 IEP/IGP The Individual Graduation Plan (IGP) should be in place prior to the Transition IEP Meeting. Based on the IGP, the IEP Team will develop the Transition Plan. The IGP will be the road map for the IEP This transition plan will serve as a guide to the development of the rest of the IEP. The IEP Team will identify the skills and supports needed to reach each transition goal which will be linked to the IGP. The IEP Team will identify the specially designed services and supports necessary for the achievement of the IGP 11/10/2018

10 Purpose &Process of Transition Planning
Preferences Section Create a snapshot of where the student is currently, their strengths, interests, career related coursework, and other need areas that relate Post-Secondary Goals Annual Goals Identify who and Identify and set annual what the student goals that help the wants to be as an student get closer to the post-secondary adult, including: goal by improving in Schooling, a job When beginning the process, you must remember that it is OUTCOME DRIVEN. This means, that we start with the desired end result, and plan backwards accordingly. As you see, each of these focuses are addressed in particular areas of the transition plan. I included screenshots of IEPonline at the bottoms just to give an idea of where this information would go. Let’s look a little closer at how each of these pieces fit into the puzzle. specific areas of need in a specific field, to reaching their goals. & living arrangements throughout the year.

11 Preparing Your Transition Plans
Is there evidence of a transition assessment? Is there mention of a course of study (current classes and future classes or curriculum)?

Is there evidence of measurable post-secondary goals for: Education/Training Employment Independent Living (if appropriate)

Do you have annual transition goals that are working towards the post-secondary goals? Measurable? Specific that relates? Do you have transition services/activities –connected to the transition goals and post-secondary goals ** Must have services/activities for Education/Training and Employment

14 Preparing Transition Plans
Use the Notification of Meeting to help determine mastery of these checkpoints Is the student listed on the Notice of IEP Meeting? Were any agency representatives invited to the meeting? If so, were they invited to the IEP meeting AND were they listed as “person/agency responsible” on the transition plan? Did the parents/student sign the Notice of Meeting which indicated an agency representative was invited?

Colquitt County, a rural community of about 40,000 is located in southwest GA. Total Schools: 14 Elementary: 10 Middle (6-7): 1 Junior (8-9): 1 High: 1 Achievement Center (6-12): 1 11/10/2018

There are approximately 9,500 students, about 1100 (11-12%) of which are SWD. Roughly 23% of our school population is Hispanic, with about 24% of SWD being Hispanic. Our free/reduced lunch rate is about 74%. 11/10/2018

All 8th -12th grade students attend and participate in their IEP meeting. Student led IEP’s are essential to successful transition. Each student is interviewed prior to the IEP meeting and then uses the interview sheet to lead his/her IEP meeting. NO STUDENT …. NO MEETING!!!!!!! 11/10/2018

Colquitt County has several transition options available to the students that are transitioning. The Graduation Coach and the Transition Coordinator work very closely to implement specialized schedules for those students that did not meet the graduation requirements, but who are returning to school. Colquitt County has implemented new programs and enhanced existing programs. The Transition Coordinator attends multiple workshops to improve the efficiency of writing a compliant transition plan. meets with each teacher individually or in a VERY small group to discuss how to write a transition plan that meets the state requirements. shares the opportunities available to students with disabilities in the Colquitt County area. 11/10/2018

35 Supervision The Transition Coordinator attends IEP meetings and views all transition plans before the are set to final. Transition plans are reviewed at the system level by the IEP Coordinator. Problem areas/specific patterns/recurring issues are addressed by delivering professional learning activities. 11/10/2018

36 Supervision cont. Transition Plans have been submitted to and reviewed by the DOE to enhance compliance. Personnel expect the plans to be inspected. Guidance for implementation of compliant plans is available and should be utilized. Supervision is critical!!! 11/10/2018

37 Key to Successful Transition
If your system believes that transition is a priority, you will see results. We are a very rural county with several options because money was allocated to make a difference in the lives of students with disabilities. Writing a plan that is 100% compliant is a natural progression if there are options in your community for students with disabilities. 11/10/2018
