2. Who we are
Synergia is an Australasian based research and evaluation company that uses analytic-driven insights to support the development and improvement of health and social services.

4. Evaluation Approach Formative evaluation
This will provide feedback and learning on the Whakakotahi activities while they are being implemented.
To support the development and improvement for ongoing implementation, future tranches, and other quality improvement initiatives.
Summative evaluation
To understand the implementation and achievements of Whakakotahi after it has been implemented over a sustained period.
The formative and summative evaluation activities will provide a robust analysis of process and outcomes for the final evaluation report.
Focus on learning from HQSC perspective about engaging primary care in QI

5. Learning across Whakakotahi
Understanding the implementation of Whakakotahi, including barriers and enablers, and the impact it is making on primary care quality improvement projects, for example, increased quality improvement capability and it’s impact on patient outcomes, equity, consumer engagement, integration, and equity.
