Sefton Community First (working title)
‘Sefton Community First’ is a joined-up approach which will engage, empower and enable our residents to be ready, willing and able to make positive changes to their lives.

Sefton Community First
Sefton Community First will be a fully integrated model that will see children, young people, adults of any age and families at the heart of what we do. Working alongside our partners, commissioned and non-commissioned services we will strive to deliver the quality the community deserves, in a more efficient and effective way. By working with people earlier we can prevent their needs escalating into more acute, intrusive and costly services.

Sefton Community First - ASPIRE
We will achieve this by delivering our proposed outcomes within our ASPIRE model which is to: Address worklessness, financial and social exclusions Support Families and Individuals in Need by providing the right support Promote Education, Training, Employment and Volunteering Increase attendance at schools, improve speech and language development and levels of progress that children and young people make Reduce Domestic Abuse, risk of homelessness and isolation Engage Children, Families and Individuals with a range of Health and Wellbeing Needs

2 FAMILIES WHERE CHILDREN NEED HELP ARE WELL SUPPORTED
(draft for consultation & engagement) Outcome 2: Families where children need help are well supported Act as lead Practitioner and co-ordinate support (single agency) Deliver group based parenting/family programmes Support and advise families who have been affected by discriminatory incidents Empower parents where there are indications of neglect to improve Support those children with young carer responsibilities and their family Support and advice for families who are carers of CYP with additional needs Act as lead Practitioner and co-ordinate support using the Team Around the Family approach(TAF) Work with difficult to engage parents and help them to access the right support at the right time Target intensive 1:1 support for parents who are struggling to promote quality and consistent care and issues of neglect Target intensive 1:1 support for Families who have CYP with a disability or health need Provide sign posting, information, advice and guidance to families and individuals Promote opportunities for families and individuals to engage and become active members of their community Promote opportunities for people to volunteer, become community champions or advocates Promote the importance of maintaining a healthy lifestyle Targeted LEVEL 2 Universal LEVEL 1 Multiple & Complex LEVEL 3 Acute LEVEL 4 Family well-being services will work with partners to reduce risks and levels of need by: Offering targeted and intensive support in the home environment or community setting Facilitate contact between children who are looked after and their parents, as directed by court Managing statutory court orders (and out of court disposals) in line with National Standards 2

SEFTON RESIDENTS ARE FREE FROM DOMESTIC ABUSE, NEGLECT OR EXPLOITATION
Outcome 5: Sefton residents are free from domestic abuse, neglect or exploitation Act as lead Practitioner and co-ordinate support (single agency) Reduce risks to young people becoming criminally exploited Support vulnerable young people and families who are on the cusp of or at risk of being involved in serious targeted crime Engage families experiencing domestic abuse Engage families to address concerns around neglect Engage perpetrators Act as lead Practitioner and co-ordinate support using the Team Around the Family approach(TAF) Engage young people through an emotional health and well being pathway to overcome the impact of domestic abuse and criminal exploitation Target intensive 1:1 support for Families to address the effects domestic abuse, neglect or exploitation and encourage capacity to change Provide sign posting, information, advice and guidance to families and individuals Promote opportunities for families and individuals to engage and become active members of their community Promote opportunities for people to volunteer, become community champions or advocates Promote the importance of maintaining a healthy lifestyle Targeted LEVEL 2 Universal LEVEL 1 Multiple & Complex LEVEL 3 Acute LEVEL 4 Family well-being services will work with partners to reduce risks and levels of need by: Offering targeted and intensive support in the home environment or community setting Facilitate contact between children who are looked after and their parents, as directed by court Managing statutory court orders (and out of court disposals) in line with National Standards 5

SEFTON RESIDENTS HAVE GOOD MENTAL HEALTH, WELLBEING & LEAD HEALTHY LIVES (draft for consultation & engagement) Outcome 6: Sefton residents have good mental health, wellbeing and lead healthy lives Act as lead Practitioner and co-ordinate support (single agency) Support families displaying signs of poor physical or mental health problems Support families with emotional stress and relationship difficulties and struggling with parenting issues Work with families exhibiting drug and alcohol misuse Increase children reaching developmental milestones Reduce the number of children who are over/under weight Engage parents with physical or mental health difficulties Act as lead Practitioner and co-ordinate support using the Team Around the Family approach(TAF) Provide postnatal support to families displaying signs of struggle or diagnosed with post natal depression Work with pregnant woman who have confirmed poor lifestyle choices Target intensive 1:1 activities to address and improve parenting capacity and inporve Relationships Provide sign posting, information, advice and guidance to families and individuals Promote opportunities for families and individuals to engage and become active members of their community Promote opportunities for people to volunteer, become community champions or advocates Promote the importance of maintaining a healthy lifestyle Targeted LEVEL 2 Universal LEVEL 1 Multiple & Complex LEVEL 3 Acute LEVEL 4 Family well-being services will work with partners to reduce risks and levels of need by: Offering targeted and intensive support in the home environment or community setting Facilitate contact between children who are looked after and their parents, as directed by court Managing statutory court orders (and out of court disposals) in line with National Standards 6

Sefton Community First – The principles
Move away from a ‘done-unto’ assessment process and instead provide an opportunity for shared conversations between the key worker and the individual or family. The approach recognises where any individual or family are now and jointly identifies the necessary steps required for them to get to where they want to be. It is a joint, time limited approach which empowers those involved to see a lasting change, which will focus on strengths . Sefton Community First will adopted a whole family approach to identify what needs to be done and what action needs to be taken The offer will promote resilience and strengthen family life through working together and where possible re-engaging with universal services

Tweaks or Re-design? Do we really understand root cause and address this or tackle the presenting factors? How do we change expectations of what the Council will provide? Public Consultation Suggestions on how we engage the public:
Proposals for the new offer, communicating changes and differences How can the Council stop creating demand through dependency? What opportunities are there to provide services which are a better fit, with what is actually needed by residents? Do residents access free services they don’t strictly need?
