HSE frequency rate calculations
1. Lost time injury frequency rate = x number of lost time injuries Total man hours worked Based on 3 lost-time injuries for YTD man hours of exposure, the company has experienced 0.77 lost-time injuries for every 200,000 man hours reached or worked. Note: 200,000 hours represents the equivalent of 100 employees working 1 full year. 2. Non Lost time injury frequency rate = x number of non lost time injuries Based on 11 non lost-time injuries for YTD man hours of exposure, the company has experienced 2.82 non lost-time injuries for every 200,000 man hours reached or worked. 3. Lost time injury severity rate = x number of lost days due to injury It enable us to tell what an average incident cost us in workdays. i.e. Based on 139 lost workdays for YTD man hours of exposure, the company experienced days lost for every 200,000 man hours reached or worked)

Safety achievements - Off mine
Koffiefontein Diamond Mines attended the 2017 MineSafe industry awards held in Johannesburg on the 1st of September 2017. MineSafe is an annual conference hosted by the Association of Mine Managers South Africa (AMMSA), South African institute of Mining and Metallurgy (SAIMM) and South African Coal Mine Managers Association (SACMA). The conferences concludes the awards function where mines are honored for their safety achievements. There are many different competition categories e.g. gold, platinum, coal, diamonds, etc. The criteria used to determine the winners of the various competitions is the Total Injury Frequency Rate (TIFR). TIFR includes all injuries – fatals, LTI’s and NLTI’s. Koffiefontein Diamond Mine received the trophy for the “Most Improved Mine in the Diamond Mining Category”. The Mine also received a congratulations letter from Leale Goddard: (Executive vice president project delivery) for DRA mineral park. The DRA Group - is a multi-disciplinary, multi-national enterprise, specializes in a total solutions offering covering virtually every aspect of mine design, infrastructure and mineral process plant design and construction.
