3 Introduction to DAART:
Welcome to DAART: DAART has a staff area/kitchen that you are very welcome to use, with a toaster,fridge and microwave If you prefer you can purchase lunch from The Mytton Restaurant. The league of friends or The Coffee shop-are all a short distance from DAART There are lockers to secure personal belongings If you are sick as well as informing the University please telephone DAART: and inform a member of staff. 23/04/18 PS

4 What is DAART ? DAART is a small Community run assessment unit based in Shrewsbury Hospital, aiming to prevent avoidable hospital admissions and support early discharge from hospital. DAART is a service run by Primary care within a Secondary care setting DAART offers an Intravenous therapy services and Blood transfusions DAART also manages the community Deep Vein Thrombosis service We offer rapid access to diagnostics and assessment these include: Blood Tests, weight ECG, BP, Pulse,Temperature X-Ray, CT Scan, Holistic assessment 23/04/18 PS

5 DAART Team: DAART is open Monday –Friday 08:030am-19:00 pm, and at the weekend and Bank Holiday DAART is open 09:00am-17:00pm offering a nurse led service for IV therapy and blood transfusions DAART is staffed by a team including GPs with a special interest in the older person known as Gp with a Special Interest’s,(GpwSI) Nurses and Administration Support. It is the aim of DAART to assess, support and involve patients and relatives in all aspect of their journey through DAART. Ensuring we explain and gain consent for all procedures and investigations. The DAART team endeavour to offer patients privacy and dignity during there time in DAART however so students are aware due to the compact nature of the unit ensuring privacy and dignity is a constant challenge and not always possible. 23/04/18 PS

6 How are patient’s referred to DAART?
Patient’s are referred in a variety of ways: GP,s are our main referral source, often telephoning on the day to make a referral. Care Coordination Centre Referrals are accepted by other health professionals both within Community and Hospital settings. Patients are able to self refer for falls if their GP is aware. Many referrals come from the falls prevention service. 23/04/18 PS

7 Learning Opportunities:
During your time at DAART you will have the opportunity to be involved with patient assessment Be involved in diagnostics and assessment Follow the patient treatment and rehabilitation journey both within the hospital and community settings Arrange to go on home visits with Occupational Therapist, and Physiotherapist. Visit department within SaTH e.g. Clinic 8 ,Scanning and Warfarin Clinic, Develop core skills: Blood pressure, ECG’s, Communication skills, Preparation of antibiotic injections/infusion and subcutaneous injections. 23/04/18 PS

We aim to provide the highest quality of care in a friendly, comfortable and safe environment, and make every effort to maintain our patients privacy and dignity. 23/04/18 PS

9 Common conditions you may encounter at DAART:
History of falls Deep vein thrombosis (DVT) Anaemia Osteoporosis Poor and decreased mobility Infections e.g. Chest infections, UTI’s and Cellulitis Cerebral vascular disease Conditions requiring blood transfusions. Memory problems Access to specialist area while on placement in DAART is available for example falls prevention, scanning department for DVT 23/04/18 PS
