State Wide AssistiveND: Medication Management Systems, Assistive Technology to keep people safe in their homes PATH: Screen youth and train staff/foster parents on safety issues, healthier foods at staff meetings, youth and families included in planning and care decisions, services follow as the level of care decreases BCBSND: Wellness activities in various communities, participate in local and state stakeholder meetings, case management focused on motivation enhancement and health literacy Behavioral Health Coalition: Narcan policies, PDMP, Good Samaritan Law, Take Back locations for unused medication, State of North Dakota: Governor Burgum’s Main Street Initiative, predictive algorithims to address at risk individuals, focus on social determinants of health, smoke free law, Recovery Reinvented, SUD vouchers, Justice Reinvestment Program (October 2017) LSSND: Staff training on ACE interface, counseling through Abound telemedicine, Restorative practices around discipline in schools, CRT diversion program. United Way: Various volunteer opportunities PSJ: Trainings on behavioral health topics in the workplace DBGR: State –wide access to child/adolescent residential psychiatric care in a trauma informed model

Cass County Fargo VA: Intimate Partner Violence program, Emergency Room Care Planning, Veteran’s Justice Program, each new or re-establishing patient is assessed for psychosocial factors that may affect their overall health, transportation assistance for low-income individuals, fees waived for low-income individuals, counseling for individuals/couples/families, services specifically for homeless vets, moral injury group, home health and community health care services, services for military sexual trauma, alternative/complimentary therapies, home based primary care , RN Care Coordinator for every primary care patient, women’s clinic, trauma processing for employees, minority veteran’s program, LGBTQ specific services, funds to help with basic needs, employees encouraged to participate in civic engagement (Combined Federal Campaign). New American Behavioral Health Coalition Re-Think Mental Health Collaborative, began their Building Compassionate Schools initiative Summer 2017. PSJ: Post-discharge follow up, IDDT, CD continuum of care, relapse prevention, partner with community providers for ancillary services (MAT), dietary/nutritional consults, financial team to assist with insurance enrollment or financial concerns Sanford: High Risk RN Case Managers for primary care patients that are struggling, Emergency Room Case Management, Medical Respite for the Homeless, Shelter RN’s in the three largest homeless shelters in Fargo. Family HealthCare: Level III Patient Centered Medical Home, 340B pharmacy, Medicaid eligible dentists, optometry (one-sight vouchers for glasses), Homeless Health Clinic, Homeless and Refugee RN Case Managers, Homeless SW Case Manager, enrollment specialists to assist uninsured patients with enrolling in the federal, MN or ND marketplace insurance plans. Centre Inc.: MAT provided by contract with Community Medical Services F5 Project: Transitional Housing, Employment Assistance, Jail meetings for men and women at Cass County Jail

VCHC: Social worker connect to resources, provides services at Housing First facility, provide DEEP at housing project, sliding fee scale, free marketplace navigator services, transportation assistance, vision program through collaboration with outside providers, behavioral health free of charge, Housing First facility RRBHS: Contract with Polk county and other counties in Minnesota to allow accepting patients from MN on 72 hour holds, DD program, medical detox regardless of BAC UND Nursing Program: LSS refugee resettlement program class project GF County: Transportation help, Medication Monitoring Program, Flu shots free for staff and clients

Central/Western ND Northland Health Centers: Increasing access through opening of 4 medical clinics and 2 dental clinics, Behavioral Health offered at all sites City County Health District (Valley City), First District Health Unit (Minot) and Heartview Foundation/Bismarck-Burleigh Public Health (Bismarck) awarded grant funding from the state for opiate overdose prevention Community Medical Services providing MAT in Minot F5 opening transitional housing in Bismarck, holding meeting at James River Correctional Center
