Headers Class sections and small groups must have headers.
(Pages 19-20) Class sections and small groups must have headers. The header information dictates the scoring. Do not combine answer sheets for a single subject under one header. Use only one header for each grade and subject.

Blank Teacher Headers Bubble all six columns for the employee ID number! Use a leading zero here Fill in your name

Answer Sheets (Pages 25-27) Pass out the correct answer sheets to the correct students. Use only No. 2 pencils. Check that students bubble their answers on the answer sheets! Make the students erase stray marks or drawings. Return answer sheets for absent students. Use the correct answer sheet for the grade tested.

The student name, grade, subject & teacher info is listed in the text box. Only the Student Number is machine bubbled. Only the student ID# is machine bubbled.

Blank MS Answer Sheets Only the Last Name, First Name and Student ID # are required for scanning. Students fill out the Test Code, Year and Form #s during the TA script.

Don’t Lose Answer Sheets!
Students should never insert answer sheets inside the ACPs! Count your test booklets and answer sheets again when signing them back in.

(Pages 24-26) Read the script verbatim as written. Practice reading the script in advance. Do not paraphrase or disregard the script.

Make-up Testing The make-up testing window is January 9-23, 2017.
(Pages 28-29) The make-up testing window is January 9-23, 2017. Headers must be created and filled out correctly for make-up tests! Testing accommodations must be administered during make-ups. Only students with excused absences can test by appointment after January 24.
