1 CORE/MHSPHP Forms Resolved Issue/Update June 2017

2 CORE Form Issue Resolved
For your information and dissemination: An issue was identified with the TSWF CORE form item for documenting "Have you received any type of care since your last visit with this clinic?" (located on the Screening tab in the "Joint Commission..." section). If the question "Is the patient deployable?" (located on the Screening tab in the "Military Specific Screening" section) is subsequently answered, the former would become unchecked.  The fix for this issue was loaded to the enterprise on June 28, 2017.  No user actions are needed and no visible changes to the CORE form will occur.

3 MHSPHP Form Issue Update FIT vs FIT-DNA
The TSWF MHSPHP AIM form has been updated to include a line for documenting FIT-DNA tests, commonly referred to as Cologuard.  The FIT-DNA test has 3-year period of "validity" for colorectal cancer screening, whereas the "FIT" test has a 1-year validity.  MHSPHP AIM form users must be careful to mark the correct test, whether "FIT" or "FIT-DNA.” Text emitted in the note:
