We use one of the largest Preferred Provider Organization (PPO) networks in the Pacific Northwest
LifeMap offers three individual dental plans:
Dollar Based Dental
Incentive 10 Dental
EPO Dental through Willamette Dental Group
Includes option to purchase Vison benefit rider
Note: LifeMap Individual Dental does not meet the pediatric mandate for the ACA

The benefit is based on a percentage of the dollar amount paid toward services.
Places dental care back in hands of the patient and the provider.
Very few exclusions (cosmetic and orthodontic only)
No age restrictions and pairs well with Medicare
Available in Oregon, Washington, Idaho, and Utah.

Eye exams can help detect diabetes, high blood-pressure, and high cholesterol, conditions that often have few warning signs.

This coverage reimburses up to $150 in vision services and/or hardware, including LASIK, per member every two years.
And, members are not restricted to a network, so they can continue to see an eye doctor of their choice.
