Clicking into the module takes you into the risk assessment options page, where you are able to choose which area within the module you would like to proceed. My Assessments This allows you to edit or view assessments that you are involved in Search Assessments You can look for risk assessments across the organisation Create New Assessment Create an assessment using a blank form Copy An Assessment Make a copy of an existing assessment to use as the basis of your risk assessment Clicking this button also gives the option of selecting a ‘best practice’ risk assessment QMUL_HS_170 MySafety Risk Assessment Guidance_23MAR2018

Create a new risk assessment by clicking on the highlighted button This will open up the risk assessment to the background / summary section You will have to scroll down to see all of the page. Clicking on an ‘Info button’ will display additional detail / guidance (where available) The clock is in the bottom right corner of the page, make sure you save the page! Background / summary section The is the page that holds basic details of the Risk Assessment, from the people involved (e.g. assessors and approvers) to information on its scope (e.g. organisation / location etc). Dates Date created - dates can be backdated Start Date - can be set to a past, current or future date Review or end date – can be set up from 1 day to 3 years*, default setting is 1 year Confidential? - if you want restricted viewing only to those you specify, click Y Assessment Title - ensure key words that enable an easy search are entered Assessment Outline - New or existing activity ? Created or revised after an incident ? Are there exemptions or defined boundaries to this activity assessment ? QMUL_HS_170 MySafety Risk Assessment Guidance_23MAR2018

Background / summary section Area responsible - From the dropdown menu, select the School / Institute / Directorate, sub-section (dept) and group / unit responsible for the activity Although you can make it non-specific, this would not be suitable for risk management at QMUL Location of risks – From the dropdown menu, select the Building (Campus), Area (Building or External Area) and Sub Area. If you need to make the location wider or have exceptions, then select Y for non specific and note the information in the Further Location Information box Documents and attachments – Upload supporting documents and/or previous version/s or note where non-electronic documents are kept Assessor – select your name from ‘look up’, you can select more than one assessor if needed. Approver – Select your line manager / supervisor or designated approver Approval summary (detail be visible later) – once your designated approver approves, this will be displayed in this box with name and date. QMUL_HS_170 MySafety Risk Assessment Guidance_23MAR2018 Distribution List – add all those who need view and sign off the risk assessment. People at risk (from activities) – select all those who may be affected by the risks identified Activities – you can click on this button to add the activities here or click at the top menu to go to this section. Once activity, hazards and risk level summary have been entered, you can view & print a summary from this button Assessment update log – when the assessment is updated, the next version will be noted as xxx/2, along with any comments by the assessor / approver. SAVE – where this appears always save before going to next section Menu options to print, copy etc are also available here. Add formal action/s to specific individuals with a deadline. Don’t forget the CLOCK ! (Session expires…)

Activities, hazards and controls
Can select / define as many activities as required For each activity can select / define a range of hazards Risk Level – referring to risk assessment matrix: Uncontrolled risk level Risk level with existing ‘in place’ controls If risk level still higher / future controls are required, option to add ‘new control’ QMUL_HS_170 MySafety Risk Assessment Guidance_23MAR2018

Select / Define Activities
If a simple task, you may have just one ‘activity’ If a complex or multi-step task, you may want to define each part or step as an individual activity Protocols, SOPs, methods can be attached Along with supplementary information e.g. safety data sheet, photographs, Equipment manual pdfs QMUL_HS_170 MySafety Risk Assessment Guidance_23MAR2018

Select / Define Hazards
Hazards can be added from the pre-set hazards for laboratory, office and other available activities A pre-set hazard can be removed If it is not relevant Information generic to hazard and existing control/s appears Info boxes give further information and links to other authoritative sources (e.g. HSD website topic page, HSE website). Select / Define Hazards
Hazards can also be created by the assessor with specific information Useful where the generic information is not sufficient or requires location or task specific detail In these created hazards, potential immediate consequences and potential long term consequences are detailed QMUL_HS_170 MySafety Risk Assessment Guidance_23MAR2018

Risk Assessment Matrix
Assists to identify the level of risk from each hazard in the activity Uncontrolled (i.e. the hazardous nature) With existing risk controls (i.e. what have you already got in place) Based on ‘Likelihood’ x ‘Severity’ Each quantified in numerical terms (scored 1 up to 5) The level of risk in both situations is quantified by multiplying ‘Likelihood’ x ‘Severity’ to obtain a score of The level of risk is then ‘banded’ into one of four* levels (*this is currently being revised into five levels with terminology changes to align with HSE model) QMUL_HS_170 MySafety Risk Assessment Guidance_23MAR2018

Approval Once you have completed all sections, click on submit for approval to your designated approver If you are not allowed further, that means you haven’t completed a section or several sections. The systems check box will pop up indicating where you still need to complete.
