5 By Viverae SimplyWell acquisition
In fall 2017, SimplyWell® was acquired by Viverae®, a wellness company based in Dallas, Texas. Joining forces was a no-brainer! We share the same mission—inspiring, improving, and simplifying wellness. So, starting November 1, Viverae® will fully rebrand to SimplyWell®. As a result, all SimplyWell® members will have access to an upgraded portal starting 9/1. We look forward to you having access to: a mobile app, enhanced education & health resources, ability to challenge your co-workers to specific peer challenges, a health center to answer all of your program questions & more!

7 September 1 starting 1. Your Desktop Portal
Your wellness portal’s URL will become connect.simplywell.com. You will need to register as a new user on the new portal. 2. The Mobile App The SimplyWell mobile app will become available. Download the new SimplyWell app from the App Store or Google Play.

8 November 1 starting The Health Center
The Viverae Health Center will become the SimplyWell Health Center. September 1 – October 31, calls will be answered as ’Viverae soon to be SimplyWell’.

health screenings Appointment Plus All screenings include: Biometrics: height, weight, blood pressure, pulse, grip strength & flex, body fat %, Labs: lipid panel, cbc, cmp, TSH. Optional tests (for an additional fee) include – bone density scan, PSA, Urinalysis, Hemoccult Fecal Occult Blood and Hearing Test

13 tips Successful screening
Avoid heavy meals, alcohol and vigorous exercise the evening before your screening. Please fast for at hours prior to your lab draw. If you are diabetic or on medication to control your blood sugar, please consult your physician before fasting. Inform the check-in person if you have not been fasting. Drink some water the morning of your screening. Do take prescribed medications that can be taken without food. Consult your physician on any questions regarding medications and this test. Bring any reading glasses needed to fill out your check-in form. Wear shoes that are easy to remove and light clothes providing easy access to your arms.

Some event credits are awarded for you. Others are able to be self-reported.

MHA and Screening are complete. View & chart your screening results. Click on the result to learn more. Members can update their health score by adding new lab results anytime throughout the program. Note, updated health scores are only to monitor progress. They will not update your incentive page or be used for reporting.

Health screenings and health questionnaires will occur annually. You must complete both of these components before the deadline to be considered a ‘wellness participant’ each year. August 31, 2018 – last date to earn credit toward the current program September 1, 2018– Portal goes Live September 2018 – Lab Draws & Biometric Measurements December 31, 2018 – Deadline for all health questionnaires (SimplyWell) and screenings (Appointment Plus) to be completed August 31, 2019 – Deadline for earning 10,000 points to be completed
