Overview We continue to face increasing costs for health insurance
Initial increase from Blue Cross was 15.6% Benefits Committee worked with our consultants to make changes to current plans to reduce increase Negotiated increase with plan design changes 8.2% 11/12/2018

On-line Enrollment Process
2009 is an active enrollment year. Every employee will need to enroll or waive coverage using the on-line tool. No paper enrollment forms will be sent out. Open Enrollment will run from November 3 through November 14. 11/12/2018

Your log-in and password will be a combination of your name and your social security number. Name: Josephine Sample SSN: log-in: samplej6789 password: (you will be prompted to change your password once you log-in) Social Security numbers for all of your dependents System provides bank level security to protect all of your information 11/12/2018
