2 Healthy in Life Clinic Mission Statement
The Healthy in Life Clinic is a non-profit organization open to the public to provide weight loss services, support and education to low income individuals and families in the greater Grand Rapids area. Our goal is to promote weight loss, healthy lifestyles and the knowledge needed for healthy meals based on a fixed income. We are committed to providing our clients the tools needed to succeed in their weight loss goals. The caregivers at the Healthy Life Clinic believe: -Every individual is entitled to weight loss services, support and education in order to be successful regardless of age, race or socioeconomic status. - Patient care will be provided with the highest quality that focuses on compassion, respect, integrity, accountability and excellence. -Obesity is not a lack of willpower, but a disease with physiological and psychological causes which require life-long treatment.

3 Obesity Epidemic in Michigan
Approximately 67% of adults and 33% of children are overweight or obese. (MDCH, 2014) Michigan ranks third worst among the states for the rate of obesity and has been among the ten heaviest states for the past 14 years. 75% of Michigan residents do not eat the recommended daily amounts of fruits and vegetables. Kent County has a 32% obesity rate for adults and a 16% obesity rate for children. (MDCH, 2011) “Obese Americans have a greater risk of developing chronic diseases, such as heart disease, diabetes and cancer” (CDC, 2012) We must act now!

4 Organizational Goals Promote nutritional education and physical activity to improve the health of the client. Understand each individual’s lifestyle differences and customize a weight loss plan. To provide a supportive and empowering experience.

5 Organizational Objectives
Assess the needs of the individual based on an initial consultation. Provide an interdisciplinary approach including a physician, nurse, nutritionist/dietician, certified personal trainer and social worker. Educate clients about the benefits of being physically active and provide examples of different exercises. Educate the client about a balanced diet and give recommendations based on income. Educate clients about triggers that may influence them to over-eat or make unhealthy choices and provide support. Schedule follow up care as needed on a case by case basis.

Assess each patient prior to physician visit. Provide medication education. Prepare specimens and send out labs. Have excellent communication skills. Must be willing to work 4-8 hours per week in the clinic Support the mission statement and philosophies of the clinic. Examine, diagnose, refer and treat patients on an out-patient basis. Prescribe medications and other treatment regimens. Review labs and other reports prior to patient arrival. Must have excellent communication skills. Must be willing to work 4-8 hours per week in the clinic.

10 Facility Needs Obtain a building through donation that includes: Small reception area needed with adjoining office space, 3-5 consultation rooms, male and female restroom facilities and some additional storage space for equipment and medical supplies A large room to facilitate meetings or exercise sessions Kitchen area for education on preparing healthy meals and cooking classes The clinic could be located outside of city limits as long as public transit is available to the clients

11 Advertising Develop a website for the clinic.
Utilize social media such as Twitter, Facebook and LinkedIn due to cost effectiveness Include content that will be considered motivational, inspirational and purposeful Share information about goals, events, health news and opportunities to become involved as a patient or volunteer Create fliers and pamphlets to be distributed to local hospitals, bariatric clinics and schools If recruiting isn’t successful within 4 weeks, the clinic will advertise in the Grand Rapids Press for 3 days, at the cost of $450. “There’s no better way to reach Michigan than by advertising at Mlive.com and to be displayed in the premiere local Michigan newspaper classifieds” (MLive, 2015).

13 Budget Proposal Healthy in Life is a non-profit, volunteer based organization with no current stakeholders Establish an advisory board, initially consisting of three group members with the option to add as the business grows Open a bank account in order to accept tax-deductible donations Hire an accountant and lawyer The RN office manager will be responsible for operating and executing the budget and is expected to be on site more hours weekly due to the amount of time the financial portion of a business requires Preparing and monitoring the budget, particularly the expense portion, is often the most time-consuming financial function of nurse managers” (Yoder-Wise, p. 223, 2011).
