What Can you and Your Family do to Help?
To help keep our health care costs lower for the 2013 plan year use your health plan wisely. Walk-in Clinic vs. Urgent Care vs. Emergency Room Preventive Care will help to keep our health care costs lower. Continue to use In- Network providers. Ensure you and your family take advantage of the in-network preventive coverage as it’s paid at 100% - no co-pays, no deductibles to meet. Be sure you and your family stay compliant with your follow up visits, lab tests and medications as directed by your physician. Get your Dental and Vision checkups!

Health Care Reform Impacts of HCR in 2011
Lifetime maximum eliminated on all plans Children can stay on Medical plan until age 26 regardless of student or marital status Pre-existing Condition exclusions eliminated for children up to age 19 Eliminate annual dollar limits on “essential benefits” Health Care Flexible Spending Account effective July 1, 2012 Annual limit reducing to a maximum of $2,500 (HCR) 2012 W-2’s Issued January 2013 Your benefit value will be shown in a new box on the W-2 Form (HCR) Future 2013 Enhanced Women’s Preventive Services (PPACA) Uniform Summary of Benefits and Coverage Document (PPACA) 2014 Pre-Existing Exclusion will no longer apply to anyone effective 2014 (HCR)

Annual Open Enrollment
The Open Enrollment period this year will offer you the opportunity to enroll in, renew, or change your Medical, Dental, Vision, Voluntary Life, Short Term Disability, Long Term Disability, and/or Section 125 coverage(s) (Flexible Spending Account and Dependent Care Account). Open Enrollment for 2013 will be held October 10 through November 9, The new benefit plan will be effective January 1, 2013. Forms will be due November 9th. Eligibility: New employees of AMERISAFE, Inc. are effective the first of the month following or coinciding with 90 days of continuous service. You are eligible if you are a full-time employee regularly scheduled to work at least 30 hours a week. Individuals may make changes or add dependents without having to provide proof of insurability on certain coverage during the open enrollment period. The open enrollment period is the only time employees may enroll in the above listed coverage without the occurrence of a qualifying event (see definition on next slide). Page 3 of your booklet

Making Enrollment Changes During the Year: In most cases, your benefit elections will remain in effect for the entire plan year. During the annual enrollment period, you have the opportunity to review your benefit elections and make changes for the coming year. You may only make changes to your elections during the year if you have one of the following status changes: Marriage, divorce or legal separation (if your state recognizes legal separation); Gain or loss of an eligible dependent for reasons such as birth, adoption, court order, disability, death, or reaching the dependent child age limit Significant changes in employment or employer-sponsored benefit coverage that affect you or your spouse’s benefit eligibility. Your benefit change must be consistent with your change in family status. Official proof of status change will be required along with the eligibility change. See your Benefit Summary for more information on Family Status Changes.

Open Enrollment is 10/10 – 11/9/12. Please turn forms in no later than Friday, November 9th so we will have all systems updated on January 1st. This year’s Open Enrollment is MANDATORY, so we need ALL employees to complete the online enrollment process. If you have forms to complete, fill out all forms completely. Now is the perfect time to update beneficiaries on your Life insurance!
