When a student gets three ‘Strikes’, the next step is to turn over the form and use the referral on the reverse side. #3

TAKE TIME TO WORK TOGETHER YOU DECIDE! Using the materials supplied, create a sample ‘Behavior Matrix’ with a partner. Place your positive behavior examples in the appropriate places on the blank matrix. Make sure you have examples for every school location and topic. (i.e. Safety, Organization, Achievement, Respect, Responsibility and Professionalism)
