Delegation/Teamwork Project
Kelli Erb RN
2
Project Overview SLC- Issues with delegation
Developing trusting relationship between RN and NT is important in successful delegation (Anthony & Vidal, 2010) Improve Communication with proper delegation Communication between team members requires constant evaluation, feedback, and modification to meet pt care goals (Cipriano, 2010) Increase teamwork, staff and patient satisfaction RNs said NTs were not completing tasks, NTs said RNs gave too many tasks. So we looked at what we could change. Being more detailed about what exactly we were doing at the time. Getting waters for patients. Improve delegation skills.

Goals and Objectives Through education and newly developed delegation and teamwork tools, the unit will improve staff and patient satisfaction and improve unit teamwork. Results will be measured through pre/post staff surveys and patient HCAHPS data What needs to change and how do we do it? Develop surveys Develop Staff Education Huddles Delegation Power Hour Analyze data Maintain Develop PPT on results

Quality & Safety Improve communication (Yoder-wise, 2014)
Prevent CAUTIs, CLABSIs, and HAPUs Improving communication is a national patient goal identified by the joint commission

Process/Progress/Completion
Staff surveys 71% took pre and 63% took post Delegation- saw some improvement Major improvement in huddles Reminder pages Accountability Power Hour! Surveys were done before the formal education which I feel helped with bias. In the survey, we specifically asked questions pertaining to the 5 rights of delegation. I think the RNs who took the survey felt they knew how to delegate, but I know before this project, follow up was not done consistently. The post data on the five rights showed improvement, but compared to the pre data it is not as drastic as I thought it would be. After the education was done, I know the act of delegation was being done a lot better. Looking at the survey data related to huddles throughout the shift, we saw major improvement. It helps that the secretaries page out the reminders and the majority of staff hold each other accountable. Power Hour has been a major change, but in the big picture, it has been extremely beneficial. Our patient satisfaction data is not dropping, but we haven’t seen a huge increase either. The pre data for patient satisfaction was about 89% which was for about 40 surveys we had received. My post data is still about 89% satisfaction, but it is only from 9 patient surveys. Hopefully, we see an increase. The whole point of power hour is to decrease the amount of interruptions the patient has during “shift change” time.

Obstacles & Challenges
Timing of power hour Going earlier 1700 mini power hour Admissions/Discharges Pulled staff Dayshift has rounds, lunches….

Ethical & Professional Issues
Who is charting for power hour? Staff acceptance Crucial conversations

Lessons Learned Collaborative leadership is assuming “a leadership role within a health care or community based care setting to promote a culture of quality and safety” (Ferris State School of Nursing, 2012). Learned how important organization is for a project manager Learned how hard it is to deal with difficult people Learned how nice it is to have staff support your decisions
9
Professional Growth HUGE growth Public speaking
Guest speaker-1E Teaching classes Increased conflict resolution skills Can’t wait for another project…already have one I am going to work on
