Social Determinants of Health
Social determinants of health are social, economic, and physical conditions that influence a person’s overall health status, such as availability of safe housing, access to healthy food, quality of education and job training, transportation options, and access to health care. Addressing social determinants of health is important for improving health and reducing longstanding disparities in health and health care.
13
Social Determinants of Health
Allegany Franciscan Ministries’ framework for understanding the social determinants of health includes five general categories: education, economic stability, social & community context, health & health care, and neighborhood & built environment. For the first cycle of FY19 Tau Grants, the three social determinants of health that will be addressed are: education, economic stability, and health & healthcare. Framework adopted by our Parent Company Trinity Health

Social Determinants of Health
Health & Healthcare Education High School Graduation Enrollment in Higher Education Language and Literacy Early Childhood and Development Access to Healthcare Health Literacy Access to Primary Care Economic Stability Poverty Employment Food Security Housing Stability As mentioned, our focus áreas for this cycle are education, economic stability, and Health & healthcare. (As a Health organization – Health will always be one of our focus áreas) I will begin to provide you with examples/ideas of projects that would align with our misión/strategies or what we have funded in the past. We are also open to innovative ideas so please dont feel restricted to the examples listed. I would love to brainstorm with you and provide feedback if you have projects outside of the ones listed. One example, an organization or a program/intiative within an organization (that of course aligns with your mission “we are a misión driven organization and we want you to stay true to your misión too) focusing on high school graduation, language and literacy that needs support with capacity building such as strategic planning or new technology equipment then you are elgible for a Tau Grant this cycle. Please remember the population served must be marginalized, under served, and/or vulnerable. Another example, a food pantry that wants to expand your services and provide Healthy food workshops or onsite Health screenings but need assistance with strategic planning . An organization that assist a vulnerable population such as exoffenders or underserved youth with job training, job placement, and/or obtaining a apprenticeship that needs assistance with website design or marketing to attract or recruit more individuals. A free Health clinic or a Health clinic that serves individuals/families that live below the FPL and need new blood pressure machines or if you are in need of strategic planning to help with patient wait times. An organization that falls under any of these categories and want to implement a Diversity Equity Inclusion intiative or program or attend a profesional conference of your liking that will increase staff knowledge If you have other ideas outside of the ones giving please feel free to give me a call so we can discuss in further detail.
