PURPOSE To inform TSMs of the health care and mental health care options available to them and their family.

SEPARATION HISTORY AND PHYSICAL EXAMINATION (SHPE)
Requirement for all separating service members. Helps identify medical conditions requiring ongoing care. Used to document medical records for disability claims. Should be started 180 days prior to separation.

DOD MENTAL HEALTH SELF-ASSESSMENT PROGRAM
Mental health and alcohol screening and referral program. For military families and service members affected by deployments. Voluntary and anonymous program offered online, by phone, or special events. Assessments for depression, bipolar disorder, alcohol use, PTSD, and generalized anxiety disorder. Gives you individualized results and resources. Active duty service members may self-refer to MTF. Active duty family members first eight visits do not require preauthorization within TRICARE network.

DOD INTRANSITION PROGRAM
Voluntary program with coaches who provide psychological health care support to service members, veterans and their health care providers during times of transition. Maintains the continuity of care for service members and veterans during transitions. Service members who have seen a behavioral health provider within one year of separation are automatically enrolled.

Connects Veterans in crisis and their families and friends with qualified, caring Department of Veterans Affairs responders through a confidential toll-free hotline, online chat, or text.
