2 Course Request Worksheets Course Requests
Step 1: Print name of both the front and back of the Course Request Form. Step 2: Review instructions on Course Request Sheet before filling out. Step 3: Check box for HSPB (top right): is it based on a 4 year University plan, or on Personalized Pathway Requirements? This box will be used to help Advisors/Counselors when reviewing course requests in regards to graduation requirements. Step 4: Fill in Personalized Pathway/Career Cluster (bottom left): This box is used as a reference when reviewing worksheets.

3 Course Request Worksheets Use Course Catalog for Course Requests
Use the course catalog to pick out course requests. Once course requests have been chosen, use the course code list to write the course codes on worksheet. The front pages of the course catalog give important information about scheduling, grading, college credit, and more. The course catalog is divided into subject sections. The index list in front of every section lists courses that CROSS-CREDIT. Also check Course Catalog Addendum for newer/edited information. Course descriptions list NCAA, College Distribution Requirements, and Pre-Requisites.

6 Course Request Worksheets Course Alternates
Alternate courses are used when there are scheduling conflicts or constraints. The scheduling team will first try to honor course requests. If those courses cannot be scheduled, then alternates are looked at next. Students who do not fill out alternate course requests on their worksheet and enter them in SKYWARD will be placed by the scheduling team. Step 1: Find alternate courses for primary course requests. Step 2: Once alternate course requests have been chosen, use the course code list to write the alternate codes on the worksheet. Step 3: Fill in Career Cluster/Focus (left side) Important Note: Students can put up to 14 course codes on the worksheet and in SKYWARD. Students can choose how many alternates they would like to enter and are encouraged to put classes they would like to take if course requests cannot be entered. Important Note: Guidelines are given of suggested alternates (i.e If student listed a Performing/Visual course request, to list a Performing/Visual Arts alternate. If student listed a P.E course request, to list an alternate in the same trimester (fall, winter, spring) and another P.E) Important Note: Alternates do not need to be listed in priority order this year.

7 Students interested in Before or After School courses
Steps Step 1 Obtain an orange “2019 Before and After School Course” sheet, fill out, and staple to Course Request worksheet. Step 2 Students will be able to enter these courses into SKYWARD just like regular day requests. Step 3 Student must sign form, and also have Advisor/CSI Instructor sign this form. 2019 BEFORE AND AFTER SCHOOL COURSES Students: If interested in taking before or after school classes, do the following: Student Step 1: Fill out this form and staple it to your Course Request Form Student Step 2: Enter any before/after school course codes into SKYWARD as you would any other regular day class. Students interested in New Market Skills Center (11th or 12th grade in 2019) 2019 NEW MARKET SKILLS CENTER FORM CSI INSTRUCTORS: Have students that are interested in New Market mark their course request sheets and also fill out this form. Step 2 Even if a student plans to take New Market Courses, they must fill out worksheet completely. Step 3 Student must see CSI Advisor and enter information on New Market Interest form.

2019 RUNNING START INTEREST FORM CSI INSTRUCTORS: Have students that are interested in Running Start mark their course request sheets and also fill out this form. Students that are missing an English or Math Recommendation: please see your Advisor/CSI Instructor. Students with questions about their Course Recommendations: please see recommending teacher.
