Annual Performance Appraisal Period Begins Conducted between 11/01 to 2/15 All employees with 6 months in position must be evaluated 2 360 Review Window Opens 11/01 to 12/31 Employee (MGR and above) completes self assessment Employee Proposes Raters Manager approves or modifies raters for employee Managers may meet with employee to discuss results when feedback is received or during performance discussion 3 Performance Evaluation Window Opens 12/01 to 2/15 This step can begin on 12/01 or upon completion of step 2. Window remains open until process is completed 4 Results Achieved 12/01 and 1/15 Employee enters results achieved, completes self assessment, then submits to Manager 5 Manager Assessment Due 1/19 Manager Completes Evaluation and Submits for Skip Level Approval (2nd Level Manager) This step can begin on 1/15 or upon completion of step 4. 6 Skip Level Approval Due 1/26 Skip Level Approval Completed This step can begin on 1/19 or upon completion of step 5. 7 Performance Discussion Goals Setting Due 2/4 Manager discussion on evaluation with employee Establish and agree on new goals, weights for goals and development plan Release of Appraisal to Employee (enter meeting date in system) This step can begin on 1/26 or upon completion of step 6. 8 Enter Next Year’s Goals Goal Weight Employee Acknowledgement Due 2/15 As discussed with manager, employee enters new goals and weights in system Manager validates weight of employee’s goals. Employee confirms meeting date in system and sends performance evaluation to COMPLETE This step can begin on 2/4 or upon completion of step 7. (*) Progress Notes 1/01 to 12/31 Employees and/or Managers enters progress notes, available throughout the year.
