6 Primary Care Regular update sessions.
Demonstration of effectiveness – quality and cost of care. Close co-operation across health and social care commissioning. Development of data packs for primary care relating to quality and quantity of EOL care.

7 Secondary and community care.
Facilitation of discharge from the acute setting e.g. planned discharge from hospital. Centralised co-ordination of care provision in the community. Guaranteeing 24/7 care. There is a need to deliver stability in community care for this to be at its most effective. Make effective use of intermediate, OoH and community services.

8 Hospice Care Extend our reach and enable hospice quality care to be delivered in any setting. Tackle inequality and widen access to hospice care. Work with communities building capacity and resilience to care at EOL. Empower a strong, dynamic and responsive hospice sector.
