3. What is the STP?
A new way of working together across health and social care
Made up of 24 organisations - CCGs, providers and local authorities
Not one, single, separate plan
It is a partnership, within which we have local plans built around places and their populations
Way of aligning the plans of all the partners

4. Why we need an STP Rising demand
GPs and local hospitals struggling to cope with demand
People are waiting longer then we want to be seen
People are living longer – and in poor health
Growing and aging population
More people living with long term conditions
Health and care inequalities
Significant health inequalities across the area
Restricted finance
Funding unable to keep up with demand

6. Who is involved in the STP?
Programme Board – made up of leaders and senior members of staff of all the partner organisations, Healthwatch, NHS England and NHS Improvement
Executive – made up of Chief Executives and Accountable Officers from organisations within the Partnership
Clinical Board – made up of senior professionals from all partners to oversee shape of future services
Finance Group – Chief Finance Officers/Directors from CCGs, providers and local authorities
Workstreams – Digital, Estates, Workforce, Communications and Engagement

7. What has been done so far?
Developed relationships and processes for working together
Started to develop local plans to transform and improve health and care at a local level
Prioritised areas to improve at a large scale – e.g. urgent and emergency care; mental health; cancer care
CCGs have agreed to work closer together to make it easier for services to be paid for and planned jointly
Working on specific projects to avoid variation and “postcode lottery” (e.g. clinical effective commissioning)

11. We face a financial challenge
Demand is increasing and funding cannot keep up
Many organisations are struggling to maintain financial plans
We need to work as efficiently as possible, for example reducing:
Reliance on expensive agency staff
Costs of ineffective treatments and wasted medicines
Duplicated back-office costs
Organisations working closer together gives best chance of better services within available resources

We have to make best use of the resources available
Avoid waste
Avoid duplication
Work differently to be more efficient
We may have to start prioritising how we spend money, based on clinical need and effectiveness
We may have to make difficult decisions in the future
No decisions have been made and all significant decisions will be made with public consultation where appropriate
