CIF MISCONDUCT/INCIDENT FORM
(Go to Officials, Misconduct Form) Pull up soccer. Password: 123 Select "Ejection Misconduct" or Incident Form Some tips to remember when completing these forms. MISCONDUCT FORM Time of Incident = 56th minute for example, or 16th minute-2nd half NOT 3:40pm There are two spaces for your name - one for your first name, and one for your second name. Address and Phone Number Site of Incident = Palm Springs HS or Recreation Park, for example, not "in the penalty area" In the drop down box for schools, the list will only include schools beginning with A. You simply need to write in the name of the school in the box provided, and the school should appear. Make sure you have the right school as many schools have similar names. If you have disqualified a player or coach from an LA City school (or any school outside of CIF Southern Section), then you indicate "non CIFSS school" in the drop down box for schools, and it will give you a space to write in the name of the school.

If the person being disqualified is a coach or bench personnel, you still must put in a number as this is a required field (let's put in the number 0 in these cases, although any number will do) Now you are given one of two options for the Disqualification. You will either check the box for "Two Yellow Cards" OR the box for "Straight Red Card". In the first case, you will be shown the two sections for reasons for Yellow Card. You must put a check in one box in each of the two sections. In the second case, you will be shown one section for reasons for a red card. You must put a check in one box in this section. The CIF Post Ejection Report is for the schools to complete. We have no involvement with this report. Action Was Against - this is a required field so even if you have two yellow cards for non physical fouls (e.g., simulating a foul, removing a shirt in excessive celebration), you can still consider this to be against an Opponent. Explanation of Each Disqualification - even though there appears to be only two lines of space in this area, you can continue to write as much as you want as the lines will drop down. Be specific as to what happened to warrant the disqualification. Had the ejected person been warned before? If you have warned the player verbally or cautioned him/her previously in the game, then you can answer yes to this question. There is no problem in answering no to this question.

Liaisons to send to: Before sending the form to your Unit's Representative, print a copy by pressing PRINT FORM at the bottom right of the form. Click SAVE to send the form to your Unit's representative. This does not save the form on your computer. That is why you need to print it first. Click SAVE and COPY if you have multiple red cards for the same game. This will send the form to your representative or liaison, and it will bring up the form once again so you can change the pertinent information. Then all you need to do is click SAVE once again and the second form will be sent to the appropriate individual. Print it first.

Misconduct - Other Notes:
The name of the school is especially important during the pre-season tournament games. Make sure all (two or three) referees get the correct school name and the city they are from Make sure you enter the minute for the red card and or the two yellows and the reason for each. INCIDENT FORM If you need to file an INCIDENT report for issues, such as fields being mismarked, dangerous spots on the field, improper uniforms, serious injuries including concussions, etc. You will click INCIDENT after you have logged in. These reports are Time of Incident = if a serious injury occurred, then put down "34th minute"; if the report is for a uniform or field issue, just put down '0'; the box has to be filled in with some number. Reporter = Referee, include address and Phone number Witness = Assistant Referee Reporter's Position = Head Referee or Referee In the drop down box for schools, the list will only include schools beginning with A. You simply need to write in the name of the school in the box provided, and the school should appear. Make sure you have the right school as many schools have similar names. There is now a drop down box to list the type of incident. Make sure you record this accurately. Be as specific as possible when describing the incident. If it is a uniform violation, note exactly what the violation was. Before saving the form, make sure you PRINT a copy. Once you press "Save" it will disappear from the screen and be sent directly to CIFSS. CIFSS sends the report onto the school.

For injuries, send an to Patrick Cary for possible concussions and serious injuries…not a sprained ankle. Name of the player, number of the player and anyone who might also have sustained or caused the injury, Also need school name and time of the incident Please Patrick Cary if you are having issues with the CIF Disqualification/Incident Report website.
